| Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection to follow up on a report submitted on 10/17/2024. Upon arrival at 12:30pm, LPA met with Director Gwen Blackmon, to whom the purpose of the inspection was explained. A tour of the facility was provided. There were children present during the time of inspection.
Census was taken. There were 25 children with 4 staff members.
Per report, on 10/15/2024, a child was playing on the outdoor play structure and fell, landing on their stomach. There were no visual injuries observed on the child, who continued the rest of the day in care. Facility was notified on 10/17/2024 that further medical evaluation took place and child was diagnosed with more serious injury.
During today's inspection, LPA reviewed the play structure and conducted interviews with two staff members. PIctures were taken and obtained from facility. Documents in the form of medical release form and messages were provided to LPA.
After review of the information above, it was determined that facility staff could not have done anything to prevent the incidents from occurring and preventative measures are being taken in order to prevent a similar incidents. Play equipment is age appropriate and in good condition.
No citations are being issued during today's inspection.
A notice of site visit was given and must remain posted for 30 days.
