32 | Personnel Records-Training: LPA reviewed three (3) staff files which includes: Personnel Record/Job Application, Health Screening, TB test, valid First Aid / CPR / AED training, Employee Rights, and Staff Training. Personnel files are kept locked in a secure location within the facility.
Client Records-Incident Reports: Client files are kept in a secure location. LPA review five (5) client files which includes: the Face Sheet, Admission Agreements, Identification & Emergency Information, Physician's Report, Ambulatory Status, TB Test Result, Personal Rights, and Assessment Service and Needs Plan.
Client Rights-Information: The facility has Personal Rights Poster posted. There are no clients in need of any postural supports.
Food Service: Clients will bring their own snacks and lunch to the day program and facility staff would assist them to heat up the food. Drinking water is available for client in the facility if needed. All toxins and cleaning supplies are secure, locked, and placed in the janitorial closet away from food supplies
Health-Related Services: Per CFO, the facility does not have any clients that take any medications. All facility staff have an updated first aid certificate and they have at least one staff able to perform CPR.
Incidental Medical Services: Currently there's no client with restricted health condition.
Disaster Preparedness: The facility has an updated Emergency Disaster Plan (LIC610D) and the last fire/disaster drill was conducted on 5/12/2025. The facility also has two temporary shelter locations.
Emergency Intervention: The Facility does not use any restraints or seclusion on clients. Clients at this facility do not require the use de-escalation techniques.
Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies observed during the visit. Exit Interview conducted and a copy of the report was provided to the Chief Financial Officer, Amy Scott.
