25 | Licensing Program Analyst (LPA) T. Tran arrived at the above licensed facility to conduct an unannounced Case Management Incident that was self-reported on 01/29/2025. Monterey Park SW Regional Office received the writing incident report on 01/30/2025. LPA met with Assistant Director, Loretta Gallegos and announced the purpose of today’s visit. Facility Representative provided LPA a tour of the facility. LPA observed proper care and supervision. All center staff that was present during today’s inspection had fingerprint cleared and associated to the designated license number.
LPA completed staff and child's record review. LPA obtained child's documents, staff notes of the incident, and a doctor's note. Interviewed were conducted with staff, children, and other. Based on the information that was gathered during today's visit, on 1/29/25, there were four staff supervised 26 children. No concerns with staff and children ratio. The incident occurred during AM outdoor play. A staff that was in close proximity to C1 (See LIC 811) observed child was going up the ladder. When staff redirected another child from going down the silde, staff heard C1 fell and immediately came to assist child. Staff also called the school for support. After assessing the injury, the facility decided to call 911 and parent was contacted. Parent was taken C1 to seek medical care. Per physician's note, child was excused from school until 3/12/25 due to having surgery and needed a spica cast on the right leg.
At this time based on the available information it does not appear this incident was the result of a Title 22 violation for lack of care and supervision.
No deficiency was cited during today’s visit. Notice of site visit was given and must remain posted for 30 days.
