The gate was closed, and no one was around the child at all or outside. About two minutes later a teacher walked down the hall and the RP told her that the child was by herself outside. Per RP, daily teachers give name tag to the kids after drop off. RP noticed the child already had the tape, name tag on her back with PH. When the teacher she told took the child into the PJ classroom, the RP walked in the PJ classroom behind her and heard one of the teachers in that room tell that teacher with the child, PH means she belongs to PH classroom. PH is another preschool classroom.
Per LPA observation of the video footage, the child was left outside alone for about 2-3 minutes (Between 8:10-8:13 AM) even while 4 staff members had walked by her while she was hiding. Per interviews it was stated that staff had talked about the incident and that yes a child was left outside after transition time. It was also stated that the child had separation anxiety and would hide in the classroom at times.
Currently, the licensee is not in compliance with California Title 22 Regulations. Therefore, there was deficiency cited during today's Inspection in accordance with the California Code of Regulations Title 22, Division 12, Chapter 1. Consultation was given on this date in English and in Mandarin.
Exit interview conducted and report was reviewed and given to Delia Castro Interim supervisor.
A notice of the site visit was given and must remain posted for 30 days.
LPA, Cynthia Reyes informed Delia Castro Interim supervisor, that this report dated 06/17/2026 document(s) (1) Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety or personal rights of a child in care. Also, LPA, Reyes informed Interim supervisor Delia Castro to provide a copy of this licensing report dated 06/17/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months (1 year) from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or written statement, must be placed in the child’s file for verification. The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Delia Castro, was provided with a copy of the LIC 9224 during this visit. |
