Licensing Program Analyst (LPA) Cynthia Chan conducted a complaint investigation for the allegations listed above. LPA arrived unannounced and met with the Administrator, Crysel Santos, to explain the purpose of the visit.

LPA toured the facility with the Administrator and inspected rooms #1, #9, #11, #22, #23, #27, #39, #45, #46, and #49. LPA obtained copies of the staff roster, client roster, a list of daily housekeeping duties, pest control service logs and service inspection reports, and the fire alarm monthly inspection report.

The investigation revealed the following:
Allegations – Staff did not ensure facility was free from cockroaches and bed bugs. During the visit today, LPA toured the facility and randomly selected 10 rooms (Rooms #1, #9, #11, #22, #23, #27, #39, #45, #46, and #49) to inspect. LPA did not observe any bed bugs or cockroaches in the rooms or around the facility.

Per the Administrator, there were reports of bed bugs last year which was taken care of immediately, but none reported recently. LPA reviewed the last pest control report which was dated 11/10/21 and showed that a full treatment for bed bugs was completed. The pest control company comes out every month to spray the dining room and kitchen area to prevent cockroaches. Based on interviews with staff, there are no more bed bugs since last year. LPA interviewed 10 clients, and none reported any bed bugs nor seen any cockroaches recently.

Allegation – Fire alarm is in disrepair. The main fire alarm is wired to the local fire department and each room consists of a smoke detector. The individual smoke detector was tested in the 10 random rooms and all were functioning properly. The Administrator stated they conduct fire drills every month and the fire alarm system is monitored monthly. LPA obtained copies of the fire drill from January through March 2022 and invoices from the Price Fire Protection company that monitors the fire alarm system. No Staff or Clients interviewed stated that the fire alarm system is in disrepair.

Allegation – Facility is dirty. During the visit today, LPA observed the housekeeper and janitor cleaning the rooms and hallways. The Administrator and Staff interviewed stated they try their best to keep the facility as clean as they could. The clients’ rooms are tidied up daily. The floors are mopped, trash is picked up, and sheets are changed weekly. Based on interviews with the clients, they stated the staff do a good job keeping the facility and their rooms clean. LPA observed the facility to be clean and well maintained.

Allegation – Facility roof leaks. Per the Administrator, there are currently no roof leaks reported. She stated the leak in the ceiling was repaired right away when she became aware of it last month and an invoice was obtained for the service on 3/15/22. The Staff interviewed also stated the repair was handled right away and there are no more leaks reported after that time. LPA inspected 10 rooms and did not observe any water damage or leak in the clients’ rooms. The client in room #23 indicated that the water leak in the bathroom’s ceiling which occurred several weeks ago was repaired right away and has not had any other leaks since then. No other clients interviewed reported leaks in their rooms.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.



An exit interview was conducted with the Administrator. A copy of this report along with the appeal rights were provided.
