Licensing Program Analysts (LPAs) Cynthia Reyes and Mariah Aguirre conducted an unannounced site inspection to present the findings of the above complaint allegation. This inspection is to ensure the health and safety standards as required by the regulations governing child care centers are met. LPA met with Arleen Pichardo, Director. LPAs were taken on a tour of the facility indoors and out by Arleen.

During the course of this investigation, LPA conducted Interviews, reviewed video footage and received and reviewed documents.

Neglect/Lack of Supervision: Per reporting party, staff did not prevent day-care child from wandering out of the facility. Around 5:30 PM on 08/28/24, a child walked out of the facility alone and that there were no staff outside with the child. LPA observed the video footage of the day of the incident and observed the child did wonder outside alone with out a staff member around.

LPA also conducted interviews with staff who stated yes the incident of the child who ran outside of the school alone did happen and the staff member was alone that day with both preschool and school age children commingled. Per interviews it was also stated that the staff member was in the back preschool class room and she had asked a couple of the school age children who were in the middle preschool class room to watch their preschool sibling and as parents of other children were picking up their children child #1 ran out of the day care and ran out the double outside gates and began to run toward the street as the parent of child #1 saw while she was in her car on the phone coming to pick up her children.

The following deficiency(s) listed on the attached LIC 9099D (deficiency page) is being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safe.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100

Exit interview conducted and report was reviewed with the Arleen Pichardo, Director.

LPA Reyes informed Director, Arleen Pichardo that this report dated 10/03/2024 document(s) (1) Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Reyes informed the Director, Arleen Pichardo to provide a copy of this licensing report dated 10/03/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A copy of the LIC 9224 was provided to the director on this date.

Per Director she will have a staff meeting to discuss Care and Supervision and will watch videos on https://ccld.childcarevideos.org/ from the child care licensing website. Director will submit a meeting agenda and what was learned from the videos and a roster of the staff who attended the training. Director and
