Licensing Program Analyst (LPA) Cynthia Reyes, conducted an unannounced annual inspection on this date. LPA met with Denise Bouchot, Teacher and a copy of the Entrance Checklist for Child Care Centers form (LIC 125) was provided. A risk assessment was conducted prior to entering the facility. This is a Preschool program which consist of 2 class rooms located in the main part of the school. The preschool program operates 6AM. to 7PM. Monday to Friday. This facility also has a school age program on site. LPA asked Denise to call the Administrator Raj Chander due to no one knows who the Director is or who is in charge. Raj arrived shortly after being called.

Teacher Denise lead LPA on a tour of the facility beginning at the front entrance of the school. Sign-In and Sign-Out sheets are located in this area. Sign-In and Sign-Out sheets were reviewed. Not all children present were signed in correctly and accounted for on this date. LPA observed that 1 of the children present is a school age child and is commingled with the preschool children. Health checks are conducted prior to the child going into the class room, which includes temperature being taken of children. Use of hand sanitizer is also available.

All areas identified on the Facility Sketch were inspected. The following was observed during the tour of the preschool class rooms, 08 children with 1 teacher in 1 class room and the other class room was set up for napping. Staff states the facility has low enrollment at this time and that the school age children use one of the preschool class rooms and not the 2 school age class rooms that are licensed.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were observed to be inaccessible to children. Raj the Administrator states there are no poisons, weapons or firearms located at the facility.

Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision and observation, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license. The Directors office which in located in the main part of the school will be used as an ill/isolation area for children. LPA observed an ill child cot/mat/chair available. Ill children will use the staff restroom located in the Directors office.

Furniture, equipment and toys were observed and inspected to be clean, safe and age appropriate and in good repair (LPA advised Raj and staff to remove any old or broken toys and furniture and to clean all inside and outside of the facility). Telephone service, heating, lighting and ventilation were evaluated.Preschool children have their own cubby to store their personal belongings. LPA observed napping equipment/bedding, and was inspected to be in good condition, appropriate storage and cleanliness. Linens are washed weekly.

Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Menus for the past 30 days are available upon request. Facility will provide Breakfast, Lunch and PM snack. Snacks and food were reviewed for availability, quantity and appropriateness to children in care. LPA advised Raj to label and date all food, dishes and bottles that are brought from home for proper storage and sanitation. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. All storage containers for solid waste, including moveable bins, need tight fitting covers on and need to be in good repair.

The outdoor playground was inspected and the play equipment need to be in safe condition, free of sharp, loose or pointed parts (Wood apparatus needs sand and paint). The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. LPA did not observe any bodies of water on the premises. There is drinking water available indoors and outdoors by use of pitcher/jugs and disposable cups. Sippy cups & water bottles will be labeled.

Page 3 Administrator Raj was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Children’s Records were not reviewed on this date for completeness; and Inspection of required forms was not made and documented on the LIC 857. LPA reviewed Staff records, which were documented on the LIC 859. Not all staff had proof against TB, measles, pertussis, and influenza, as well as current Mandated reporter.

There is at least one person trained in EMS (emergency medical services) Pediatric CPR and First Aid present during this inspection. Pediatric CPR & First Aid is documented for staff who have it and is current for the Director.

Medication: This facility provides Incidental Medical Services – IMS. No children at this time to review storage of medication and equipment/supplies, and no review of children’s, personnel, and administrative records. Children's roster was reviewed and is not current and number of total enrolled at this time was documented. Disaster drill log was available and posted visibly for all to see. The last drill was conducted on 11/2022.
First Aid supplies were observed in each classroom and were observed to contain the required first-aid supplies. According to Staff, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. No transportation is being conducted by the facility for preschool children.

Update: Effects of Lead Exposure and the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD. were discussed.

Carbon monoxide detectors and smoke detectors are present and are in operable condition in the facility. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. LPA observed they are both located in each classroom, as well as the fire extinguisher type 2A-10BC and the valve on each indicated that they are all fully charged and each has a tag with a service date of 06/29/2022. Fire extinguishers need to be serviced annually or as often as necessary

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. This inspection will need to be continued for children's record review and citations.
A notice of site visit was given and must remain posted for 30 days.

Upon receipt of a report documenting a Type A deficiency or substantiated complaint, the licensee shall do the following:

1. Health and Safety Code Section 1596.8595 (c) - A licensed child care facility or home shall provide to the parents of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as specified in paragraph (1) of subdivision (a) of Section 1596.893b. Failure to comply with paragraph (1) shall result in an immediate civil penalty of one hundred dollars ($100).

2. Each licensed child day care facility shall post a copy of any licensing report pertaining to the facility that documents either a facility inspection or a complaint investigation that results in a citation for a violation that, if not corrected, will create a direct and immediate risk to the health, safety, or personal rights of children in care. Failure to comply with paragraph (1) shall result in an immediate civil penalty of one hundred dollars ($100).

3. Upon enrollment of a new child in a facility, the licensee shall provide to the parents or legal guardians of the newly enrolling child copies of any licensing report that the licensee has received during the prior 12-month period that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as set forth in paragraph (1) of subdivision (a) of Section 1596.893b.

4. The licensee shall require each recipient of the licensing report described to sign a statement (LIC 9224) indicating that he or she has received the document and the date it was received. The Acknowledgement form (LIC 9224) must be maintained in each child’s file immediately upon receipt from parent. A copy of the parent Acknowledgement of Receipt of Licensing Reports Form was provided during this inspection.
