Licensing Program Analyst (LPA) Cynthia Reyes conducted a unannounced Plan of corrections (POC) inspection on this date to ensure the Type A and Type B deficiencies cited on 04/12/2024 have been cleared. Upon entry LPA observed the Director Arleen alone in the center with 5 preschool children in care and Aid/Van driver Elizabeth arrived shortly after, coming from shopping for supply's. During this inspection staff member Diana, book keeper arrived and assisted the LPA with documents needed. Administrator Raj Chander arrived to the facility around 11AM to assist with the school age program as the school age children arrive early on Wednesday.

LPA observed the following citations listed below have been corrected. A POC letter has been printed and given for the corrections made and cleared.
(Type A) Criminal Record Clearance: Staff #4 is now fingerprint cleared and staff #1 is now fingerprint associated.
(Type A) Drinking water tested for lead: Testing has been completed and pending Results.
(Type B) Buildings and Grounds: The front gate has been fixed.
(Type B) Fixtures, Furniture, Equipment and Supplies: Steps of the climbing apparatus are fixed with cushion material.

The items below are still pending corrections and have been recited on this date.

(Type A) Indoor Activity Space for School Age Children:

The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with and given to the Director Arleen Pichardo.
