25 | Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection, to follow up on an incident that was reported to the Department on 03/27/2026. Upon arrival at 2pm, LPA met with Facility Representative Gabby Baldovinos, to whom the purpose of the inspection was explained. A tour of the facility was conducted and census was taken. There were 13 children with 3 staff members.
On 03/26/2026, Child #1 was having a tantrum and threw items near them. A hard dinosaur toy and a thick handheld whiteboard were thrown and struck Child #2 and Child #3, resulting in injuries to their heads, requiring medical evaluation. During today's inspection, LPA conducted interviews with two staff members, and conducted phone calls with two additional staff, and one parent, not present on site.
During interviews with staff, both staff stated the Child #1 has displayed these behaviors before when they are upset. Staff #1 states that they try their best to keep all children safe. The Options For Learning program has reduced the hours of Child #1 and has added Behavioral Specialist to their schedule daily, except Fridays, while at the facility. LPA contact Parent #1, who stated that they had to take Child #3 to the Urgent Care in order to have them evaluated the day after the incident, due to Child #3 waking up with a headache.
LPA reviewed Urgent Care note for Child #2, clearing them to return to care. LPA also obtained copy of completed Individual Child and Family Support Plan for Child #1. LPA observed Child #1 and their Classroom Behavior Specialist (CBS) during inspection. The CBS had to remain next to Child #1 to help keep them attentive to activities, snack time, and classwork. Child tried to run around the classroom multiple times during visit. Per Adult #1, they are working with family of Child #1 to get Applied Behavior Analysis (ABA) done for them.
