25 | Licensing Program Analyst Cynthia Reyes and Mariah Aguirre conducted a Case Management Deficiencies inspection, to address deficiencies revealed during a previous inspection on 10/03/2024. This inspection is to also ensure the health and safety standards as required by the regulations governing child care centers are met. LPAs met with Diana Rodriguez, staff manager and explained the reason for the visit. When entering the facility, second staff member Elizabeth Cervantes was present. Shortly after, new director Monique Smith arrived at the facility and was also informed of the reason for the visit. Census of Children was taken.
During today's facility tour, LPAs did not observe any school age children in care at time of arrival.
During the previous visit on October 3rd, 2024, LPAs observed citations regarding limitations on capacity due to school age and preschool age children commingling, building and grounds, several electrical outlets were observed to be uncovered, Director Monique Smith placed covers on outlets during inspection. Also observed were multiple backpack hooks to be broken with the nails sticking out of the wall, chipped paint was observed on several of the walls in the classroom, and the carpet in the main school age classroom was observed to be stained all over. Building and grounds. due to the cabinet of the sink in the main school age classroom was broken, and cleaning supplies were accessible to children due to no safety lock being used. Building and grounds due to an exposed water fountain that is not in working condition outside of the school aged area and was observed to not be covered. Physical plant, the short gate at the entrance of the facility was observed to be not in proper working condition as it does not close completely at all times. LPAs also observed a toilet in the school age bathroom not working properly.
