Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced Required Annual Inspection and met with Program Director Jordan Quenun. LPAs disclosed the purpose of the inspection and was granted entry into the facility by Program Director.

LPA Calderon alongside with Program Director J.Quenun conducted a tour of the facility. The facility has an After School Program serving physically and/or developmentally disabled children ages five 5 to 18 years of age. The facility can also operate age 18-22 under SB309. The hours of operation are: Monday through Friday 1:30pm to 6:30pm. The facility also operates a full day program during Spring Break and Summer Break from 8:30am to 5:30pm.

It was observed by LPA an Adult Program operating on-site, per Program Director J.Quenun program is operated by Adult and Senior Care Licensing Department. Per Program Director, stated school age uses the shared space with the After School program however, the programs do not commingle and have different schedules when using shared space. The shared space that is used are: classroom space, outdoor activity space, art room space, swimming pool space, lockers, small gym room space and restrooms. Based on file review, LPA observed the facility has conducted shared space activities (during different hours of operation) for multiple years.

All areas identified on the Facility Sketch were inspected. School age Room during observation had (0) zero children and (0) zero staff present. LPA inspected the following: The school age room, pool, outdoor play yards (2), the art studio, kitchen and restrooms. LPA observed facility was observed in good repair and cleanliness. Furniture and equipment were also inspected for age appropriateness and good repair. LPA observed telephone service, central heating/ ventilation, and lighting. Age appropriate restrooms, sinks and toilets were inspected for availability, safety and sanitation. All restroom are in operable condition. Storage areas for chemicals components and cleaning supplies were observed locked in storage room in the School Age Classroom and in the a storage closet in staff restroom near the building entrance in Main Office Building. Per Program Director there are no poisons are stored at the facility.

Per Program Director Quenun, there are no weapons at the facility. Per Program Director Quenun no children take naps during the after school program. Per Program Director Quenun, facility does provided transportation when needed. Per Program Director Quenun, there are no School Age Children who currently take medication at the facility. Per Program Director Quenun, there are medications stored in the Program Resource Room located in the main building for the Adult Program and if School Age Program were to have children on medications, medications are stored in the same location. LPA observed room to be locked and medication are in a locked medication box mounted against the wall. Medication room is inaccessible to children and only able to enter with a key card.

LPA observed outdoor area, there are two areas for outdoor play. The area with the play equipment was observed to be free of sharp, loose or pointed parts. The outdoor activity space surface is maintained in a safe condition and appears to be free of hazards. Areas around and/or under climbing equipment have cushioning material to absorb a fall. The other outdoor area is a free space grass area that also is used for gardening. LPA observed two fountains accessible for children to use in the outdoor hallway area.

LPA observed a large indoor swimming pool located in a physically separate building. During time of visit doors automatically opened. Per Program Director, doors are automatic only when lifeguard is on duty and when no lifeguard all (4) entrances to the pool are remain locked. The School Age Children use pool with supervision and LPA noted pool days on the activity schedule, posted in the School Age Classroom. LPA during file review reviewed that facility was Licensed in effective January 2, 1993. LPA strongly advised Program Director to ensure that the facility provides 100% supervision during any and all activities near the pool area and during pool activities. LPA informed Program Director that regulation states: If licensed prior to June 1, 1995, child care centers with existing pool fencing shall be exempt from the fencing requirements specified in Section 101238(e)(1) until such fence is replaced or structurally altered. Per Program Director, when asked if the pool structure has an any altering changes Quenun stated no changes as far as their knowledge. Pool pictures were taken. (pg 2/ cont..)

LPA reviewed snack menus that are posted at least one week in advance and visible to authorized representatives. The facility provides a PM snack. LPA observed Kitchen area were food and snack preparation are conducted. Area was observed to be clean and having proper equipment.

Per Program Director Quenun he facility has a interconnected system for smoke detectors, fire pull system and carbon monoxide detectors, devices are interconnected with Long Beach Fire Department.

Per Program Director inspections are done by Long Beach Fire Prevention Bureau and Apple Valley Communication Co. LPA observed 2A10BC fire extinguishers throughout the buildings, the fire extinguisher in the School Age Classroom was observed to be serviced: December 2024. LPA reviewed Disaster and Fire Drill schedule, it was reviewed that the last drill was conducted on: 10/17/24.

Facility uses paper sign in and sign out forms to ensure children are being properly signed in and signed out.

LPA reviewed (4) staff and (4) children records. Program Director Quenun has up to date Shallow Water Lifeguarding with AED/ CPR for Professional Rescuers and First Aid certification on file. LPA reviewed Lead Sampling Water Lead Testing Report conducted by Adviro, tested December 21,2022. Results show all water fixtures passed.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Due to insufficient time, LPA will return at a later time to discuss water testing, conduct staff interview and to continue children and staff file review.

During the exit interview, the Facility Representative Jordan Quenun, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
