On 05/13/2025 Licensing Program Analyst (LPAs) Amelia Morales and Laticia Thompson met with Jayne Rosen, Director. The purpose of the visit was an unannounced case management – licensee Initiated inspection at the facility to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were 150 children and staff 41 present during this inspection. This is an application to increase the capacity to a total of 300 preschool children ages 2 to 5 years. The child care center will operate Monday – Friday 7:30AM -4:00PM (office hours:  7:30AM -4:00PM).  The fire clearance was approved on 03/24/2025 by LA City Fire Prevention Bureau). LPAs toured and measured the on-limits areas. 

Census: Room #1: there were 12 children with 2 staff; Room #2: 12 children with 3 staff; Room #3: 13 children with 3 staff; Room #7: 10 children with 3 staff; Room #8 : 11 children with 2 staff; Room #9: 11 children with 2 staff; Room #20: 10 children with 2 staff; Room# 21: 11 children with 3 staff; Room #22: 13 children with 3 staff; Room #23: 11 children with 3 staff; Room #24: 11 children with 3 staff; room #25: 11 children with 3 staff; Room #52: 14 children with 3 staff; Room #54: 14 children with 3 staff; Room #B: 10 children with 3 staff.

On 5/07/2024, LPA Silva Garibyan measured the indoor Rooms: 1,2,3,4,7,8,9, 20,21,22,23,24,25 , 52, and 54. As well as the outdoor activity space. The previous measurement for the areas were:

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Classroom measurements are:

The Indoor total measurements contain a total of 7,628 sq. feet, which will accommodate Licensees request for 210 children.

Outdoor Play measurements are:

The outdoor activity space total measurements contain a total of 16,687.5 sq. feet, which will accommodate Licensees request for 210 children.

On 5/13/2025, LPA Morales and Thompson measured the newly constructed building for the following rooms: A-10, A-11, A-12, A-13, A-14, A-15, A-16 and Art Studio. As well as the newly constructed outdoor play area.

Classroom measurements are:

The Indoor measurements contain a total of 4790.79 which will accommodate Licensees request for an additional amount of 90 preschool children.

Outdoor Play measurements are:

The outdoor activity space measurements contain a total of 8831.40, which will accommodate and additional 117 children.

Preschool children will have access to 13 sinks and 7 toilets.

The following was observed. Carbon monoxide detector are installed in all classrooms. First aid kits were available upon inspection, in each classroom. Mats were observed in each classroom, they are located in the classroom cabinets. Age appropriate equipment was observed in good repair. There were filtered water dispensers in each room next to the sink faucet. The school has central heating and cooling. Windows were in good repair free of chipping paint, dirt, insects or debris. Adequate lighting was observed. The classrooms were clean and in good repair. Disinfectants and cleaning solution and other toxins or poisons were made inaccessible to children.

The facility was equipped with a working telephones inside the classrooms. Staff shall sign children in, and parents or authorized adults will sign children out. The required postings were also posted in a common area for parents and visitors review.

The facility provides daily snacks, and children bring their food from home

The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. LPAs informed Director, that any changing table must be in arms reach of a sink. Age appropriate toys and equipment were observed in good condition. No hazardous conditions or equipment was observed during today’s visit.

LPA observed shade and benches for resting. Based on today’s inspection the facility shall be recommended for a capacity of 300 children, determined by the applicants request.

There is an in-ground swimming pool that is fenced per regulation to make it inaccessible. The pool gate is self-latching, self-closing and opens away from the swimming pool. Swimming pool is not used for preschool children. LPA emailed new AB: requirements to Director.



Notice of site visit was given and must remain posted for 30 days.
