On 8/22/2024 Licensing Program Analyst (LPA) Lisa Clayton conducted an unannounced case management inspection for the purpose of ensuring licensee has completed the Plan of Correction issued on 08/09/2024. LPA was greeted by Licensee Yasmeen Chunara. LPA toured the facility inside and outside and observed 45 children being supervised and care for appropriately by 8 fingerprint cleared staff.

LPA Clayton observed parent signatures on 50 LIC 9224 forms for children currently enrolled and parent signatures on 6 LIC 9224 forms for student who are scheduled to begin attending the CCC in September 2024. Per the Admin Assistant, the parents of all other newly enrolled students received the report and LIC 9224 via email and are expected to return the signed form prior to the first day of school.

Licensee has completed the plan of correction.

No deficiencies were cited during today’s visit in accordance to the California Code of Regulations Title 22, Division 12, Chapter 1.

An exit interview was conducted and a copy of this report, POC Letter and Notice of Site Visit were provided. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
