32 | Based on admission by the facility staff the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
A Type A deficiency was cited and Civil Penalties assessed, according to Title 22 of the California Code of Regulations and/or Health & Safety Codes.
The LIC 9224 form was provided and explained that a copy of this report must be posted for 30 days, and a copy must be given to the parents of each child currently enrolled and for any child enrolled within the next 12 months, with a signed copy of the LIC9224 placed in the child’s file. If this is found to not be completed the facility will be cited and assessed a civil penalty.
An exit interview was conducted, a copy of this report along with a copy of the Civil Penalty assessment, Appeal Rights and Notice of Site Visit were issued.
The Notice of Site visit must remain posted for 30 days.
