25 | An unannounced Case Management-Incident inspection was conducted on this date by Licensing Program Analyst (LPA) Silva Garibyan to follow up on an Unusual Incident which allegedly occurred on 05/01/24 and was reported via phone on 05/03/24 to Community Care Licensing. LPA arrived at the facility and met with Rosaura Castillo, Center Coordinator who guided LPA on a tour of the facility. There were 23 toddlers, 18 infants, nine toddler and nine infant staff present at the time of the visit.
The incident that occurred on 05/01/24 was reported to the Department via telephone on 05/03/24. The Unusual Incident which was reported concerns the possible violation of the Personal Rights of a Toddler (C1).
During the inspection conducted on May 17, 2024, LPA Garibyan conducted interviews with the Center Coordinator and three staff. Based on the interviews conducted, there was no evidence to support the allegation reported. No additional follow up is necessary regarding the incident reported.
No deficiencies are being cited at this time.
Center Coordinator Rosaura Castillo was provided with The Notice of Site Visit (LIC 9213) – which must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
