25 | Licensing Program Analyst (LPA) Warren Birks conducted an unannounced Case Management Incident inspection. This inspection is regarding an incident that took place on May 9, 2023. LPA met with Director Alec Colchico who provided information and assistance during the inspection.
During the inspection, LPA interviewed child #1, staff #1, staff #2 and Director Colchico. Staff #1 indicated staff #1 witnessed the incident and guided LPA to the playground area (where child #1 was accidentally injured). Staff #1 indicated she was approximately 10 feet away when child #1 climbed a small tree (branch is approximately four(4) feet from the ground). When child was asked to come down, the child slipped and fell sustaining a injury to the right arm. Staff #1 walked child #1 to Staff #2 who provided first aid.
Staff #2 indicated that staff #2 was in a different area of the playground and did not witness the incident. Staff #2 informed LPA based on attendance records that day, there were 18 school age children with three (3) staff. LPA observed the tree area to be clean, free of abnormal hazards and appearing to be in safe good condition (with grass surrounding). The tree does not appear high enough to warrant that the tree be off limits. However, LPA is advising the facility to not have children climb the trees.
Note: The child was taken to the doctor for medical treatment. Staff followed the required protocol in terms of first aid and "reporting requirements" as the May 9, 2023 incident was reported to Child Care Licensing within the required 24 hours.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Director Alec Colchico. |
