On 01/26/2023 Licensing Program Analyst Jose Calderon conducted an un-announced Annual Inspection, with an emphasis on infection control, using Care Tools Program. The Day Program, Advocacy for Respect and Choice Long Beach is located on 4919 E Stearns St Long Beach CA, located on ARC grounds, in which other programs are run, but not licensed by CCLD.

LPA Calderon met with Administrator Arielle Neal, and the purpose of the visit was explained. Upon Arrival LPA Calderon observed Covid-19 postings, and Mask guidance on the Outside Entry doors. Staff and Consumers are screened for Covid-19 symptoms before entry. A symptom log is kept for Consumers.

During the Physical Plant tour LPA Calderon observed the following licensed areas: Administrative offices, employee lounge with washer, activity room's: Gallery room, Library room, Studio room, Boutique Room, PE Room, Art Room, and outdoor patio/Lunch area. The licensed wing provides three (3) men restroom and (3) women restroom, Restrooms are all clean and operational. Adequate lighting was observed throughout the facility. LPA Calderon observed bathrooms were found to be within Title 22 regulations and were clean and operational. LPA Calderon noted the following: Bathroom #1 hot water temperature properly measured at 109 degrees Fahrenheit, and bathroom #2 hot water temperature properly measured at 107 degrees Fahrenheit, and bathroom #3 hot water temperature properly measured at 105 degrees Fahrenheit, Kitchen hot water temperature properly measured at 113 degrees Fahrenheit. The program has ample storage space for staff and clients. The lunch area was clean, and a refrigerator was available for client use. Clients bring their lunches daily, however the program keeps back up snack/food available, in the event a client forgets their lunch. The facility walkways throughout the day program were clear of hazards and all exits were clear of debris. 6 Fire extinguishers were observed to comply, serviced last on 03/08/2022.

LPA Calderon toured the outside grounds and observed a shaded area with tables and chairs. LPA Calderon reviewed (3) client and (3) staff files which comply. Fire alarm (hardwired), Smoke detectors (15) and carbon monoxide detectors (2) were observed to working and in compliance. The last emergency disaster drill was conducted in March of 2022.

Commercial General Liability Policy #2023-00104 policy period from 01/01/2023 to 01/01/2024 underwritten by Cypress Insurance Company, coverage 1,000,000/3,000,000 is valid at time of inspection. LPA Calderon spoke to Administrator Arielle Neal who will email a full copy of insurance contract to LPA Calderon by 01/31/2023. All the required documents are posted in the facility in a clearly visible area to all clients and guests. LPA Calderon reviewed LIC500 and noted all staff associated to facility per LIS. The facility does NOT handle resident's money/cash resources and no Surety bond is needed.

During the visit, LPA Calderon observed the facility infection control practices. LPA Calderon did observe screening protocols for visitors, staff, and residents, sanitizing stations (Located in common areas and restrooms). LPA Calderon observed staff were wearing face coverings, an isolation room is confirmed for the facility. LPA Calderon observed the facility has a 30-day supply of Personal Protective Equipment (PPE).

LPA Calderon advised the Administrator Arielle Neal to continuously monitor the Centers for Disease Control (CDC) website and Community Care Likening Provider Informational Notices (PIN) for any updates relating to COVID-19 guidance.



During today’s visit there was no deficiencies under California code of regulation title 22, division 6, chapter 8.

Exit interview was held and a copy of the report was provided to the Administrator Arielle Neal by hand
