On 01/18/2023 at 11:45 AM an Unannounced Required – 1 Year Inspection was conducted by Licensing Program Analysts (LPA) Katrina Chicote and Franchesca White. Facility is currently licensed for a capacity of 44. Licensing staff met with Nancy Perez, Coordinating Teacher (CT). The program currently operates a Part Time Program programs AM Program: 8:00 AM to 11:00 AM and PM Program 12:00 PM to 3:00 PM Monday thru Friday. LPAs singularly toured facility both indoors and outdoors. Facility is located on Lincoln Elementary School campus in RM B-1, with other room B-2 currently not being used at this time due to staffing. LPAs observed required licensing postings on Parent Board in classroom.

At the initial start of the site visit, there is a total census of 14 children and a total census of two teachers present. LPAs observed children arriving for PM program during visit.

Beginning at 11:48 AM, furniture and equipment were inspected for age appropriateness and good repair. LPAs observed flooring and carpets to be free of hazards at time of inspection. LPA observed child size tables and chairs available for children well maintained. LPAs observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Per CT, children do not nap at facility due to part time program offering. Availability of drinking water was reviewed. LPAs observed water coolers available for children and children's personal water bottles. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPAs observed two bathrooms available in classroom, one with one toilet and one sink and the other two toilets and one sink, totaling three toilets and two sinks.

Snack/lunch menus were reviewed and observed posted. All meals and snacks are provided by the Long Beach Unified School District. Per CT, AM Program is provided breakfast and PM Program is provided lunch. LPAs observed staff pick up lunch from cafeteria at time of inspection. A first aid kit is kept in classroom and inventoried for supplies. At 11:57 AM, carbon monoxide detectors were tested and smoke detectors observed present in the facility. Fire extinguishers have been serviced on 08/02/2022. LPAs observed disaster drill log available, last drill conducted on 11/30/2022. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. LPAs observed these items stored in under cabinet sink that CT opened with a key. Coordinating Teacher states that there are no poisons stored at the facility. At 11:59 AM, LPAs observed medicine that is stored in classroom for C1 to have required documents and to be stored per Title 22 Regulation requirements.

Beginning at 12:02 PM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed play structure with stairs and slides that has cushioning material to absorb fall that is all well maintained. LPAs observed bicycles, plastic playhouse, sensory tables (sand and water), art materials, and other age appropriate toys available for children to use outside that is well maintained. Required shade and fencing were inspected. LPAs advised CT that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water bottles and water coolers during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed electric sign in/out sheets and all children present signed in at time of inspection. Personal Rights of children were observed by LPAs.

Beginning at 10:20 AM, staff and children’s records were reviewed. Children’s Records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857. The review of Staff records was documented on the LIC 859.

Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. LPA observed Pediatric First Aid/CPR expires on 08/27/2024 for S1. LPA did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office .All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed with the Coordinating Teacher:
Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today only advisories were given on 01/18/2023.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
