On 9/13/2022 Licensing Program Analysts (LPA)s Judy Laureano and Deborah Lowe conducted an unannounced Annual Required Inspection at 1210 S. La Cienega Blvd., Los Angeles, CA 90035. LPAs met with Director, Chaya Rotenberg and toured the facility indoors and outdoors. LPAs observed 28 children and 7 staff member providing care and supervision. Facility currently operates Monday through Thursday 8:05 a.m. to 4 p.m. and Friday 8:05 to 1:00 p.m.

Preschool facility has a Toddler Option that is NOT currently used. Based on director interview, toddler option is not being used. Facility is licensed for 43 preschool children and 7 Toddlers (18 months to 30 months).

LPAs toured and inspected the facility in accordance with the facility sketch. Director confirmed that the following rooms are currently being used: Gesher Room, Nursery Room A and Nursery B. Room 4 originally used for the Toddler Room, is currently not being used. LPAs inspected Room 4 and did not observed any children in the space. Director confirmed that facility has not used the Toddler Option has not been used in the last 8 years, approximately since 2015.

There is no swimming pool or other bodies of water on the premises.

There are no firearms or ammunition allowed or stored on the premises. Director confirmed that facility has an armed guard walking the outer perimeter of the property. Armed guard does not interact with preschool children. Facility has submitted a request for a firearm waiver. The copy of the armed guard permit was reviewed and a copy was provided to LPAs. LPAs observed armed guarded outside the gated facility.

Disinfectants and cleaning solutions are kept downstairs in the storage closet outside Nursery A. Facility has a designated cleaning person during the day and a cleaning crew in the evening. LPAs observed 2 cleaning bottles outside Nursery A/ Room 6 and a Type B citation was issues. Director corrected the citation and ensure the cleaning solutions were made inaccessible to the children in care. No poisons were observed during the inspection.

Medication is stored in each classroom unless medication needs to be refrigerated. Director stated that all medication that needs to be refrigerated will be stored in the director’s office. Currently the Gesher room was observed to have medications store in the storage closet that is kept closed. LPA's reviewed facility's First Aid kits and observed all necessary items.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts.
The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPAs observed a metal grate outside the sandbox area that needs to be repaired. A Type B citation was issued and Director covered the area, making it inaccessible to children until the metal grate can be replaced. LPAs observed children’s size bicycles that need their pedals replaced and Director made them inaccessible. Bicycles will be stored until they can all have their pedals and/or handle bars replace.

All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors, filter water is provided via water pitcher to children in care. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents, pest control services comes monthly. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.

Capacity and limitations as specified on the license are being maintained.
At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at off site activities. Director’s CPR was taken on 6/18/2022. LPAs reviewed staff’s file and files were found to be incomplete. LPAs issued a Technical Assistance regarding the Flu Vaccine and/or Waiver. LPAs provided director with a copy of LIC311A and LIC 125 to use as a reference. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department.
The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. LPAs observed the sign in/out currently being used does not record the time of day. Director agrees to revised facility's sign in/out sheet to ensure time of drop off and pick up is recorded. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

LPAs reviewed 10 children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment.

Menus are posted in every classrooms and are provided to parent via email and/or newsletters. LPA’s discusses the importance of maintaining menus for at least 30 days and noting any changes and/or substitution made.

Facility representative, Chaya Rotenberg, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) currently being provided. Licensee is aware that an IMS
plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.

LPAs and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with the licensee facility representative, Chaya Rotenberg.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiencies are being cited: (see next page, 809 D) Licensee was provided a copy of their appeal rights.
