Licensing Program Analysts (LPAs) K. Dunlap and F. White arrived at the above facility for the purpose of an unannounced Random Annual inspection. LPA Dunlap announced the purpose of the visit and was granted entry into the facility by Licensee, Carol Sanders. There are 7 children enrolled, and 3 preschoolers present at the time of inspection. All adults in the facility have current criminal background clearance. Per Licensee, the hours of operation are 6:30 a.m. to 6:00 p.m. (Monday - Friday). Disaster Drill Log was available during the inspection. Last drill was conducted 6/1/2024. Facility rosters was reviewed and was current. Parent Board was observed to have License, Facility Sketch, Parent's Rights, Facility Roster, Emergency Disaster Plan, and Fire Permit.

At approximately 10:50am LPA Dunlap observed Parent Board does not have LIC 613A, Menu, PUB 269.

Children's roster was reviewed and is current. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Children present were signed in.

At approximately 11:00am, LPA Dunlap observed the Licensee and one staff in Room 2 with 3 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. All children were observed to be under visual supervision of a teacher at all times.

At approximately 11:10am, LPA Dunlap observed furniture and equipment in Classroom 1 and 2 was age appropriateness and in good repair. LPA observed material and equipment to be free of sharp, loose, or pointed parts. LPA observed that Classroom 1 and Classroom 2 has block area, dramatic play, manipulative play, art area, and library area. Telephone service, heating, and lighting were evaluated and are operable. at the Licensee's home. Napping equipment (mats) were observed.

At approximately 11:20am LPA Dunlap observed disinfectants, cleaning solutions, and other items that are dangerous to children, were observed to be inaccessible to children. LPA observed those items kept in the locked off-limits office in a cabinet above a storage space. According to the Licensee, medication is only administered to a child when accompanied with a doctor's note. Per Licensee, no child enrolled requires medication to be administered. Licensee understands that storage areas for poisons must be locked not just inaccessible. Facility has one or more functioning carbon monoxide and smoke detectors that meet statutory requirements. First Aid supplies were observed in the classroom.

At approximately 11:25am LPA Dunlap observed that water is readily available indoors via a water cooler.

At approximately 11:30am LPA Dunlap observed all kitchen areas/food preparation areas and food storage areas are not kept clean and are not free of litter, rubbish, rodents, and/or any other vermin. The center provides breakfast, lunch and p.m. snacks. The meals and snacks are prepared on site. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. LPA observed the following:

· Perishable food stored refrigerator in containers and did not have a label with date

· Stove Top (Stuffing Mix For Turkey) box food stored on cabinet shelves expired on May 2023

· 2 packages of Hot Dogs stored in freezer had frost bite

· Jar of mayonnaise stored on shelf in refrigerator had a knife left in closed jar

· Molded vegetables in bottom crisper bin of refrigerator

This poses an immediate risk to the health and safety of children in care. A Type A will be issued.

At approximately 12:00pm LPA Dunlap observed the outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe

condition and is free of hazards. Playground 1 has slides, climbing structures, tricycles, playhouse, basketball court, and have cushioning material to absorb a fall. Playground 1 has adequate shade, tricycles, balls, manipulative play, and jump ropes. LPA observed that water is readily available outdoors via water bottles, and outdoor water fountain. The Licensee states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Licensee states there are no weapons or firearms on the premises.

At approximately 12:20pm LPA Dunlap observed the 2A10BC Fire Extinguisher with date of 1/5/24

At approximately 12:10pm LPA Dunlap reviewed 3 children files. LPA observed that 2 out of the 3 files did not have the following documents, and/or missing information:

· LIC 995

· LIC 613A

· LIC 627

This poses an immediate risk to the health and safety of the children in care. A Type A will be issued.

At approximately 12:45pm LPA Dunlap reviewed 3 staff files. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. LPA observed that the following documents were missing, missing information, and/or expired

Criminal Record Clearance - Child Care Centers

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Lead Testing – Child Care Centers (CCC)

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

Megan’s Law - On this date, 7/2/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
