Water Cooler for children's use is inoperable

INVESTIGATION FINDINGS:

Licensing Program Analyst (LPA), Portia Bowden conducted an unannounced complaint inspection to conclude the investigation of the above allegations. Upon arrival, LPA met with Lead Teacher Candler in classroom B2. LPA observed 16 children in care supervised by 3 staff.

Based on LPA observation on 10/7/25 there is a large hole in the floor in the entryway of classroom B1. Based on observation and interviews conducted 10/31/25 the water cooler in classroom B1 is inoperable. Based on the preponderance of evidence, the allegations Water Cooler for children's use is inoperable and there is a large hole in the floor located in the entryway of classroom B1 were substantiated. A finding means that the complaint is substantiated, and the allegation is valid because the preponderance of the evidence standard has been met.

Two Type B deficiencies were issued during today's visit.
Exit interview conducted with Teacher Candler and a copy of this report and appeal rights were provided. A Notice of site visit was given and shall remain posted for 30 days.

Per Teacher Candler she will ensure that there is bottled water for each child daily in the classroom. Per Teacher Candler watercoolers in both classrooms were fixed on 11/3/25.
