Licensing Program Analyst (LPA) Elka Chavez conducted an unannounced Required 1-Year inspection. LPA met with Lead Teacher, Julieanna Garcia who guided LPA on a tour of the facility. Director Telma Bayona arrived at 10:15 AM to conclude inspection with LPA. This is a one room classroom facility located on the campus of Longfellow Elementary School in room K1. This is a preschool program licensed for 24 children. Per the Lead Teacher there are 16 children enrolled. There are two sessions 8:00 AM - 11:00 AM and 11:45 PM - 2:45 PM, Monday through Friday.

At 10:00 AM all areas identified on the facility sketch were inspected. Upon arrival, LPA observed 14 children with 2 staff. Teacher-child ratios were observed to be in accordance with Title 22 regulations. The facility is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. Staff files were not reviewed during today’s inspection. The files are kept in the main office located at Tibby Preschool. LPA was advised to make an appointment to review staff files. All children were always observed to be under visual supervision of a teacher.

Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean.LPA observed 6 small carperts with nonslip material located along the center activity wall. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the nurse’s office is used as the designated ill isolation are. Parents are contacted immediately when children are determined to be ill. A first aid kit is kept in the classroom. Smoke detectors are present and fire extinguishers have been serviced in December 02, 2021.

At 10:05 AM menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Breakfast and lunch are provided by Longfellow Elementary through

the Compton Unified School. Children have their own personal drinking containers that are labeled with their name. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. Facility has one or more functioning carbon monoxide detectors that meet requirements.

At 10:11 AM LPA inspected the outdoor activity space, equipment and surface to ensure the area is maintained in a safe condition and is free of hazards. LPA observed the cushioning around the climbing equipment on the left side of the playground is cracked. LPA was able to fit her left foot inside of the crack that runs across the play equipment. At 10:13 AM LPA observed the play structure located on the right side of the playground was missing a plastic cover in one of the climbing areas. Teacher stated that the children are currently using the playground located on the left side to avoid a child from falling through the whole in the play structure. At 10:16 AM LPA observed exposed pipes along the wall where children play. LPA observed children running towards the wall and staff #1 tell the children to be careful. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. At 9:50 AM LPA observed the license posted inside the classroom (1010) known as K1 used by the preschool. At 10:56 AM LPA observed children are picked up and dropped off at the gate located in the Elementary School next to room number 1150. LPA observed the gate is kept locked with a padlock.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director Telma Bayona was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Director will get bids and provide copies to the department. The material used by the vendor started to split within 2 years and the director will make sure that they don't use the same contractor to replace the cushion material. The staff will report it to the principal and to the chief facilities officer, we will document, take pictures and have it repaired.
