An unannounced Required one year Inspection was conducted on this day by Licensing Program Analyst (LPA) Raul Navarro. Facility is currently licensed for a capacity of 46 School Age children. Licensing arrived at 02:00pm and met with Facility Shannan Cheffen. The program currently operates Monday thru Friday from 2:00pm to 6:00pm. LPA observed 25 children in care with three staff.

LPA toured the School Age classrooms and outdoor play area. Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings was observed. Availability of drinking water was reviewed. The facility has not had the water tested for lead. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. Materials for the children were observed to be clean.

A first aid kit is kept in the classroom. LPA observed a carbon monoxide detector and smoke detector in the big room. Fire extinguishers have been serviced. Hazardous items including poisonous cleaning compounds were stored inaccessible to children.

Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Required shade, drinking water and fencing were inspected. LPA Navarro advised that the children need to be within the direct care and supervision, including visual supervision of the teachers at all times.

Children refill their water from a filtered water system. LPA also observed bottled water available for children in care. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Children are provided an afternoon meal. The food is brought in from an outdoor vendor. Food preparation areas were toured for safety, cleanliness and proper equipment.

Children’s Records were reviewed for LIC 700- Identification and Emergency Information, LIC 702- Child's Preadmission Health History, LIC 627- Consent for Medical Treatment, and LIC 995A Notification of Parents’ Rights. Sign-in and out procedures were reviewed.

Staff records were reviewed for LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.



Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.



Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The deficiency listed on the following page were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809-D. The deficiency that are being cited need to be cleared to protect the children’s health & safety.
