On 1/13/2025 Licensing Program Analysts (LPA) Judy Laureano conducted an unannounced case management inspection at the above-mentioned facility for the purpose of ensuring that standards are being met in accordance with California Tittle 22 Regulations and California Health and Safety Codes.

LPA met with Debbie Mendell, Director, and toured the facility both indoors and outdoors. LPA observed 11 children in care with 2 staff member providing care and supervision.

LPA Laureano reviewed 10 children’s file and observed 4 files did not have immunization records and 3 out of 10 files did not have a signed physician's report. Director agrees to ensure that the 4 files missing immunization record and the 3 files missing their physician’s report is completed by 1/31/2025. LPA cited 2 Type B citations

2 deficiencies were cited during today’s inspection in accordance to the California Code of Regulations Title 22, Division 12, Chapter 1 and California Health and Safety Code.

Upon on receipt of this report, the Licensee shall post the Notice of Site Visit.
The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.

An exit interview was conducted, and report was reviewed with Director Debbie Mendell. A copy of this report and appeal rights were discussed and left with the Licensee, whose signature on this form confirm receipt of these documents.
