On December 8, 2022, Licensing Program Analyst (LPA), V. Wheatley conducted a case management. LPA was greeted by Staff #1 Nori Henson who was observing parents sign their children out today.

Nori Henson called the licensee Ethel Henson who stated she was on her way to the facility to meet with the LPA. The licensee did not come to the facility however sent her son, Wymon Henson a.k.a. Monty to meet with the LPA. Mr. Henson provided a tour of the entire preschool.

During the inspection, LPA reviewed fingerprint clearances for the staff and observed Staff #1 does not have a criminal record clearance. The licensee was informed on 10/13/22 that all adults working on the premises are required to have a Criminal Record Clearance.

In addition, during the inspection LPA observed a child go to the restroom unsupervised inside of the main building of the office. LPA learned that the child walked alone from a classroom.

Both of these deficiencies are violations according to Title 22 Regulations.

Licensee will ensure that all employees working on the premises are fingerprint cleared prior to working or being on the premises. The licensee will submit a plan of correction to the Department by 12/9/22. The staff member #1 will not be allowed on the premises until fingerprint clearance is received.

101229(a)(1) -Responsibility for Providing Care and Supervision-The licensee shall provide care and supervision as necessary to meet the children's needs.No child(ren) shall be left without the supervision of a teacher at any time. Supervision shall include visual observation.

Licensee will ensure that all children are supervised at all times. Licensee will submit a Plan of Correction to the Department by 12/9/22. The licensee will have a meeting with all the staff and submit the meeting agenda to the Department as well as the roster of all teachers that attended the meeting.
