On 10/23/2024, Licensing Program Analyst (LPA) Lisa Clayton conducted an unannounced Annual Required Inspection for the school age license for STAR – Mar Vista. LPA met with Jenifer Bautista Site Director and Colbie Witherspoon Co- Site Director. The facility operates Monday, Wednesday, Thursday and Friday 2:30pm to 6:00pm and Tuesday 1:30pm to 6:00pm. LPA observed 39 children in the E-TK, TK and kindergarten program and 31 children (1st – 3rd grade) for a total of 70 children in care, being supervised and cared for appropriately by 9 fingerprint cleared staff.

LPA Clayton toured the campus inside and outside for a Health and Safety inspection.

The Program operates on the Mar Vista Elementary School campus. The program currently operates in the following areas: bungalow 37, bungalow 38, kindergarten class 25, the cafeteria and the yard. Site Director stated the children do homework and arts & crafts in the classrooms, and sports, dance and recreation on the yard. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. All materials and surfaces accessible to children are toxic free. The isolation area is located in bungalow 38 (the Star office). LPA inspected the outside play area and surface of the outdoor activity space is maintained in a safe condition and is free of hazards.

The facility provides hot supper (evening snack) to children in care provided and prepared by Mar Vista Elementary staff. LPA Clayton toured the kitchen and inspected the food storage areas designated for the after-school program. Menus are posted at least one week in advance.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. LPA observed multiple first aid kits in the Star office (bungalow 38).

The children have access to 1 boys bathroom & 1 girls bathroom inside of the main building, and a girls/boys bathroom inside of kinder class 25. All toilets and handwashing facilities are in safe and sanitary operating condition.

Drinking water is available inside and outside. The children bring in their own refillable water bottles, but cups are available if needed.



Facility was observed to have a centralized smoke detector/alarm system. LPA Clayton observed a working carbon monoxide detector in bungalow 38. There is a fully charged 3A40BC Fire Extinguisher in each classroom.

LPA Clayton reviewed 5 children’s files and observed files to have current contact information of parents and authorized representatives, and authorization for medical treatment, signed Parent’s Rights and signed Personal Rights receipts.

LPA Clayton reviewed 5 employee files which were found to be in compliance with immunization for childcare providers and LPA Clayton observed current Mandated Reporter training certificates in addition to CPR/First Aid as required for opening and closing staff.

Site Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.



Incidental Medical Services (IMS) policy was discussed, and LPA Clayton provided Site Director with an IMS Plan sample for her reference. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA Clayton informed Site Director of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, no Deficiencies were cited, Technical Advisory was issued (see LIC 9102).


During the exit interview the report was reviewed and a copy was provided to Site Director Jenifer .
LPA Clayton posted a Notice of Site Visit which must remain posted for 30 days.
