25 | On 12/22/2022 at 9:15 AM, Licensing Program Analyst (LPA) Katrina Chicote conducted an unannounced case management inspection to follow up on an incident that was reported to the Department on 12/16/2022. Upon arrival, LPA announced purpose of visit and met with Amy Payne, Coordinating Teacher, who granted entrance to facility and provided LPA a tour of the facility inside and outside. Census was taken.
On 12/16/2022, an incident was reported to the department in regards to upcoming construction to facility floors which will result in closing the facility from 12/26/2022 - 01/06/2023. The facility reported this incident to the Department within the required 24 hours and submitted a written report within seven days, complying with Title 22 Regulation reporting requirements.
During this inspection LPA interviewed Coordinating Teacher in regards to facility's plans for construction and accommodations made for families and staff. Coordinating Teacher states floors in hallways of facility will be replaced but classrooms will not be affected at this time. LPA observed area where planned flooring will be replaced at time of inspection. Per Coordinating Teacher, Full Day program is consolidating to another CDC site while AM Program will be off for Winter Break. Coordinating Teacher states parents were informed verbally during pick up/drop off as well as written notices sent out. LPA obtained copies of written notices at time of inspection.
Based on the information obtained and LPA observations, the facility provided notices to parents in a timely manner and have provided reasonable accommodations for children and staff. No deficiencies will be cited today.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
