An unannounced Required – 1 Year Inspection was conducted on 12/22/2022 at 10:30 AM by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 88 but currently have 42 enrolled. LPA announced purpose of visit and initially met with Amy Payne, Coordinating Teacher, who granted entry to facility. The program currently operates two programs a Full Day Program from 6:30 AM to 5:30 PM and an AM Program from 8:00 AM to 11:00 AM, Monday thru Friday. LPA was taken on a guided tour of the facility of both indoors and outdoors. Required postings were observed in facility hallway at time of inspection.

At the initial start of the site visit, there is a total census of 35 preschoolers and a total census of six staff present. The facility is made up of three classrooms (RM 301, 302/303, and 304), but per Coordinating Teacher, only two of the rooms are being used at this time due to low enrollment. LPA observed 17 children with three staff (S1, S2, and S3) in RM 301 and 18 children with two staff (S4 and S5) for RM 302/303. LPA arrived while RM 302/303 were playing outside.

Beginning at 10:30 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. Telephone service, heating, lighting and ventilation were evaluated in each classroom. LPA observed learning stations for dramatic play, art, and Science and Math manipulatives with shelves filled with child appropriate toys free of loose and sharp parts available for children. LPA observed quiet play area with soft seating and books available for children. LPA observed floors and carpets to be well maintained and free of hazards at time of inspection. There were cubbies observed as storage for children's belongings. Cots were inspected and appropriate storage of blankets were observed. Per Coordinating Teacher, linens are provided by the facility and taken home weekly by the parents. Availability of drinking water was reviewed. LPA observed water coolers and individual water bottles with children's names labeled available for drinking water. paper, area safety and sanitation. LPA observed each classroom have access to their own bathrooms, Bathroom 1 has four toilets and two sinks and Bathroom 2 has two sinks and two toilets. LPA observed bathrooms are also accessible from outside play area. LPA reminded Coordinating Teacher that bathrooms should have full visual supervision, including visual supervision, at all times.

A first aid kit is kept in each classroom, which LPA observed is taken outside when classrooms are playing outside. LPA observed carbon monoxide detectors are operable and smoke detectors are present in the facility. Fire extinguishers have been serviced on 06/21/2022. Disaster drill log was available during inspection, last drill conducted on 12/01/2022. LPA observed sick/isolation area is in Teacher's Lounge. Hazardous items including cleaning compounds were stored inaccessible to children. LPA observed these items stored in cabinet in Kitchen and per Coordinating Teacher, cleaning compounds are also stored in locked file cabinet in each classroom.

At 10:55 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed outdoor play area being used at time of inspection and observed adequate supervision. LPA observed large outdoor play area with play structure that has slide, steps, and climbing equipment that is age appropriate with cushioning material below to absorb fall all well maintained. LPA observed concrete bike path with bicycles and other ride along toys available for children as well as shelves with Lincoln logs, puzzles, and other age appropriate toys available for children free of loose and sharp parts. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water bottles during outside play, which LPA observed at time of inspection. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Snack/lunch menus were reviewed and observed posted. At 11:00 AM food preparation areas were toured for safety, cleanliness and proper equipment. Per Coordinating Teacher, they get food delivered from LBUSD Nutrition Services Kitchen and they only prep the food for distribution at the facility. Facility serves breakfast, lunch, and snack

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA.

All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District.

Beginning at 11:10 AM, children and staff records were reviewed. Children’s records were reviewed for completeness and documented on the LIC 857. Staff records were documented on the LIC 859. Staff present have proof of the AB 1207 Mandated Reporter Training certificate and proof of completion of Pest Management on file. There is at least one staff member trained in CPR and Pediatric First Aid present during this inspection. CPR card expires on 03/12/2023 for S3. LPA did not review confidential forms and information due to the files being stored at the administrative office.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA observed medicines for two children (C1 and C2) in RM 302/303 stored according to Title 22 Regulation requirements at time of inspection.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited but advisories were provided on 12/22/2022.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
