25 | Licensing Program Analysts (LPAs) Susann Sanchez and Ashley Calderon conducted an unannounced case management inspection to follow up on several incidents that was reported to the Department. Upon arrival, LPAs met with Coordinating Teacher, April Tucker who provided LPAs a tour of the facility inside and outside. Census was taken.
On 09/12/23, an unusual incident report was made to the department regarding a portion of the outdoor fence is going to be replaced. Construction took a couple of days. During this time the area was blocked off and inaccessible. Based on LPAs observation, the fence on the perimeter was replaced. There was a gate between the grass area and the pavement located on the left side of the playground was removed from the playground area so children have more outside play space. Play area has a fence to secure safety. LPA's took photos to update the file.
On 02/01/24, an unusual incident report was made to the department regarding Child #1 took a fire extinguisher off the wall and threw it on 01/31/24. During this investigation LPAs interviewed staff. During incident there was 2 staff members and one other child. LPAs took photos and measured where the fire extinguisher is. The fire extinguisher measured at 2 1/2 feet from the ground up. Administrator Cristabelle Rodriguez stated that Long Beach Fire Inspector Edward Martinez is scheduled to come out and move up fire extinguisher so it's 4 to 5 feet higher to ensure the safety of children in care.
On 02/08/24, an unusual incident report was made to the department regarding an incident that occurred on 02/07/24, April Tucker, Coordinating Teacher stated that Child #1 opened the main door of the facility and walked 3 steps out the door. At this time, Child #1 had 4 staff members assisting Child #1 so child was always supervised. Adult #1 quickly grabbed child hand and quickly guide them back into the facility. Administrative staff is currently working with Child #1 guardian and have developed a plan to better assist |
