1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced annual inspection on March 2, 2023 at: 12:15PM. LPA met with Laura Sidney, Site Supervisor, who guided analyst on a tour of the facility. This is a preschool program which consists of 3 classrooms; RM #1 Berry Bears (ages 4-5 years), RM #2 – Bunnies- Not in use due to low enrollment, and RM #3 – Ewoks: (ages 3-4 years). Facility operation hours are Monday to Friday from 7:00am to 6:00pm. All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room #1-Berry Bears: Staff #1 and 5 children and Room #3- Ewoks: Staff#2 and 7 children. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and in good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Laundry is done as needed washing cot sheets and blankets. Napping equipment is stored in the closet in hallway in a separate area. Per Site Supervisor, the isolation area is in the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Site Supervisor states that poisons are locked in the cabinets upstairs in bathroom and it is locked and inaccessible to children. Carbon monoxide detectors were observed and are operable. The center has a kitchen located next door to the center at 2230 Olive Ave. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags and shall be discarded immediately after each meal. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.

At approximately 12:30pm, during inspection of the play yard in back, LPA observed an area located directly where you step down to enter the play area, looking to the left: an area that is open and provides access to the side of the center. The area also leads to next door to the kitchen (2230 Olive Ave). The house has a board of wood attached to the bottom half of house and that extends outward creating an open crawl space about 3 feet wide. These areas are obscured from supervision. LPA recommends the area to become inaccessible to children by placing a barrier. LPA took photos. Green Storage locker located in playground area (left side) in front of kitchen home-(2230 Olive): The door is broken and won’t lock or close and is accessible to children. LPA recommends repairing and making inaccessible to children. Availability of outdoor drinking water was observed via water coolers and cups. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free . There are no firearms stored on the premises. There are no pools or bodies of water at the facility. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children’s Records were reviewed. (The name, address, and telephone number of child’s authorized representative is on file) and medical Assessment.) for completeness; Inspection of required forms was made and documented on the LIC 857. LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present did have proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPA also issued the Review of Staff records (LIC 859) to the licensee during this inspection. The LIC 857 and the LIC 859 documents the staff and children’s files that were reviewed during this inspection. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 01/31/2023. Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides Breakfast, Lunch, and PM snack. First Aid supplies were observed in the office in a first aid cabinet and children’s restroom. Per Site Supervisor, medications are not currently being provided. LPA advised Site Supervisor to contact LPA if they need to update an IMS statement. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is in the office.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Per Site Supervisor, Laura Sidney, Lead Testing was completed on 12/01/22. Dr. Khaleghi, Executive Director submitted the completed lead testing reports and licensing forms. LPA conducted the lead exceedance inspection on 03/02/23.

Corrections will be completed by 04/01/2023.

Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the Site Supervisor, Laura Sidney.

The following deficiencies listed on the attached deficiencies page are being cited in accordance with California Code of Regulations Title 22.
