25 | On 02/02/2023 at 9:20 AM, Licensing Program Analysts (LPAs) Katrina Chicote and Franchesca White conducted an Unannounced - Case Management inspection to follow up on an incident that was reported to the Department on 01/18/2023. Upon arrival, LPAs announced purpose of visit and met with Cecilia Verduzco, Coordinating Teacher, who granted entry to facility. LPAs observed 13 children with two staff (S1 and S2) during inspection.
On 01/18/2023, an incident was reported to The Department by Facility Representatives, where a parent alleges their child's personal rights were violated while in care. The purpose of the inspection was to conduct interviews of children in classroom where alleged incident took place. LPAs interviewed six children (C1 - C6) and additional staff during inspection.
Based on the information collected during multiple inspections and interviews conducted of staff and children, there is not enough preponderance of evidence to substantiate the reported allegation. Therefore, there were no deficiencies cited on this date.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
Exit interview was conducted and report was reviewed with the Facility Representative, Cecilia Verduzco.
