On 03/15/2023 at 1:20 PM an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. Facility is currently licensed for a capacity of 72. LPA announced purpose of visit and initially met with Bertha Mercado, Teacher, who granted entry to facility. Facility is located on Muir Elementary School campus in RMs 28108 and 28107. The program currently operates a Part Time program in both classrooms from 9:30 AM to 12:30 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors. LPA observed required licensing postings on Parent Boards in each classroom.
At the initial start of the site visit, there were no children present at time of inspection and two teachers (S1 and S2).

Beginning at 1:25 PM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed floors and carpets throughout to be free of hazards at time of inspection. LPA observed child size tables and chairs available for children well maintained and learning stations for dramatic play, math, art, and science with toys well maintained free of loose and sharp parts and a library area with soft seating available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Per Teachers, facility does not offer nap time due to part time program offering. Availability of drinking water was reviewed. LPA observed water bottles available for children in each classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. At 1:32 PM, LPA observed two bathrooms both classrooms use to be accessible right outside RM 28108. LPA observed one bathroom to have one toilet, one urinal, two sinks and the other bathroom to have two toilets and two sinks, totaling 3.5 toilets and four sinks available. LPA observed additional sinks 7 available in each classroom, totaling six sinks available for the facility.

Menus were reviewed and observed posted at time of inspection. All meals and snacks are provided by the Long Beach Unified School District. Children are provided with a full lunch. Per Teachers, staff picks up lunch from cafeteria and children eat inside classrooms. Trash cans used to discard food have tight fitting lids.

LPA observed a first aid kit is kept in each classroom and inventoried for necessary supplies. At 1:25 PM and 1:37 PM, carbon monoxide detectors were tested and observed operable in each classroom. LPA observed smoke detectors observed present in the facility and fire extinguishers have been serviced on 07/14/2022. LPA observed disaster drill log available, last drill conducted on 02/27/2023. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. LPA observed these items stored in high cabinet in classroom with operable child safety lock. Per Teachers, there are currently no children requiring medications, LPA observed locked medicine cabinet in classrooms to be empty. Teacher states isolation area for sick children is in Main Office.

Beginning at 1:29 PM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed multi-level play structure with three slides in outdoor play area that has cushioning material to absorb fall that is well maintained. Required shade and fencing were inspected. LPA observed various benches and seating areas available for children under shade. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water bottles and drinking fountains in outdoor play area. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios and care and supervision to meet the basic needs of children were discussed with staff present. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were discussed.

Beginning at 1:55 PM, staff records were reviewed. The review of Staff records was documented on the LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate and Pest Management on file. LPA observed Pediatric First Aid/CPR expires on 12/2023 for S2. LPA did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 03/15/2023.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
