Licensing Program Analysts (LPAs) Jeanette Estrada and Joshua Ortega conducted a case management visit at the facility to follow up on an unusual incident that was reported to the Department on 10/8/24. LPAs initially met with Teacher Penny Smith and informed her of the reason for the visit. Program Coordinator Cristabel Rodriguez joined at a later time. There were a total of 49 children and 6 staff present during the inspection.

During this inspection LPAs conducted staff interviews.
LPAs will return for further investigation.

Exit interview conducted with Facility Representatives. A copy of this report and a Notice of Site Visit were provided. Facility Representatives were advised Notice of Site Visit must remain posted for 30 days.
