On 8/8/2024 at 12:45 pm Licensing Program Analyst (LPA), Claudia Kam made an unannounced required visit at above licensed facility. LPA disclosed the purpose of the inspection and was granted entry into the facility.

LPA met with director Cynthia Sanders and was taken on a guided tour of both indoors and outdoors of the facility. LPA observed Teacher-child ratios, and staff names recorded. All center staff that was present during today’s inspection had fingerprint clearance and are associated to the designated license number. Care and supervision evaluated it is determined that the basic needs and personal rights of children are met and appropriate.

The hours of operation are 6:30 am – 6:00 pm, Monday - Friday. Facility is currently licensed for a capacity of 96. At the initial start of the site visit, there is a total census of 48 children and a total census of 7 staff present. The facility is made up of 4 classrooms. Room 1: had 22 children and 2 staff, Room 2: had 12 children and 3 staff, Room 3: 14 children and 2 staff, Room 4: is currently not in use at time of inspection. Room 4 was observed to be vacant and not in use. LPA observed all posting requirements for operation on the posting board and the following documents were observed LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menus, and Activity Schedule.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children napping equipment met the requirement regulation at time of inspection. All bedding is washed weekly by the facility if the items are soiled during the week the parents will take the soiled items bring in clean bedding. Storage for children's belongings, bathrooms facilities, and isolation areas were inspected. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. Bathrooms have 3 child size sinks and 3 child size toilets for classroom 3.

Classrooms 1 and 2 each have a bathroom with 3 child size sinks and 3 child size toilets. LPA observed all classrooms to have cubbies with wooden toys, stacking toys, craft supplies, puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. LPA observed carpets and age-appropriate tables and chairs in each classroom. LPA observed a large first aid box kept in each classroom. First Aid supplies were inventoried and were found to be in compliance with regulation.

Facility provides breakfast and lunch, menus were reviewed at time of inspection. All food items are prepared by the facility. Food items are properly stored. Per director the snacks are provided by the parents and each child brings a snack in their labeled lunch box or bag and they are kept in the labeled cubby for each child. Food preparation area was toured and found in compliance for safety, cleanliness, and proper storage. Drinking water is provided via a water delivery service. A water dispenser was observed in each classroom with disposable cups for drinking. Carbon monoxide detectors and smoke detectors are present in the facility. LPA confirmed detectors at time of inspection and were determined to be in operable condition. Fire extinguishers were observed as serviced on 10/31/23. Last fire drill was conducted on 5/22/2024.



At 1:30 PM, LPA observed outdoor play area which consists of 2 covered patios with tables for activities, a climbing structure, plastic play structures, basketball hoop, tricycles, ride on toys, and tunnels. Outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. Required shade and fencing were inspected. LPA advised Director that the children need to be always within the direct care and visual supervision by the teachers. Per Director, the children have access to drinking water via and outdoor water dispenser with disposable cups. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

LPA reviewed with director the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. LPA observed CPR and First Aid Certification for center staff. CPR card expires on 7/2024 for Staff Members.

Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. to ensure the most current information regarding regulations for all those who assist in providing care at the facility.

The following were discussed with the Director:
The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCLD within the time frame specified by the regulation.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Center was reviewed for lead compliance and center does not meet standards for lead. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.



Based on this information the following deficiencies on the LIC 809 D are being cited today 8/8/2024.

A notice of site visit was given and must remain posted for 30 days.
