This is an amended report to reflect timeline of events during the investigation.
On 5/27/26 Licensing Program Analyst (LPA) Angela Luz met with Facility Representative Ludalla Clayton to conduct an unannounced complaint initial visit regarding the above-mentioned allegations. LPA explained the purpose of the inspection. LPA toured the facility indoors and outdoors, and observed 5 staff supervising 24 children at the time of the tour. The facility is within ratio and capacity compliance today. LPA called and spoke with Licensee Christina Evans about the purpose of the visit.

On 4/14/26 The El Segundo Regional Office (ESRO) received complaint with the above mentioned allegation. The reporting party alleged that staff did not prevent a child (Child 2, C2) from harming another child in care.

On 4/17/26, LPA made initial visit to open the complaint investigation. LPA conducted staff interviews, made observations, and obtained copies of incident reports, facility roster, personnel report, letter sent to families and receipt confirmation, and email correspondence. LPA requested videos from the facility. Licensee sent LPA the videos on 4/30/26 and 5/4/26. On 5/27/26, LPA made a subsequent visit and conducted an interview with Licensee, made observations, and delivered findings.

Videos revealed staff keep C2 near them throughout the day. LPA observed staff stay near C2 and that C2 plays independently and with peers. LPA observed C2 and another child playing together and hugging. Record review revealed the facility does not write incident reports if one child hurts another child. The facility writes incident reports when a child has an incident or injury. LPA advised facility to begin writing incident reports for incidents involving 2 or more children to track behaviors.

Interviews revealed that challenging behaviors at the facility are a known issue. Facility-wide communication has been sent out to families. Staff use talking with the child, teaching the child problem solving skills, and redirection as ways to manage challenging behaviors. If needed, staff will have a meeting with the family regarding their child's behavior and come up with a behavior plan. Staff work with families to ensure communication and consistency between the facility and home. Staff also provide resources regarding challenging behaviors. There are currently no children with written behavior plans.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Ludalla Clayton in person and Licensee Christina Evans via telephone call. Amended report reviewed with Licensee Christina Evans in person.
