25 | On 12/1/2025, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. LPA met with Licensee Ollie Mae Allen and she was informed of the purpose of today’s visit.
The facility is licensed to care for (6) clients, of which (6) may be ambulatory, (4) non-ambulatory. The facility is licensed to serve developmentally disabled or Mentally Ill adults ages 18 through 59. Currently, there are four (4) clients. None of the clients have Restricted Health Care Conditions, and none are utilizing postural supports or protective devices.
The facility is a single-story home located in a residential neighborhood. The property consists of the following: 3 client bedrooms, a staff room, 2 common bathrooms, living room, kitchen, dining area, washer/dryer, detached garage, and an outdoor shaded area on the front porch.
The bathrooms were in good condition and operational. The water temperature ranged from 105°F to 118. °F, and the bedroom temperatures ranged from 72°F to 78°F the facility appeared to be clean, sanitary, and appropriately furnished. LPA observed cleaning supplies / toxins stored in the house in a locked closet inaccessible to clients.
The kitchen was inspected, and there was a 5 day supply of perishable food items and a 7 day of non-perishable food items which was adequately maintained. The file extinguishers were fully charged and operable. The last Fire/Disaster Drills were conducted on 11/10/2025.
