On 02/22/2024 at 9:15 AM an unannounced Required – 3 Year Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Claudia Kam. Licensing staff announced purpose of visit and met with Facility Representative (FR), Elvira Rodriguez, Director, who granted entry to facility and provided guided tour of facility both indoors and outdoors. The program currently operates an Infant Program with a Toddler Option Monday thru Friday 7:30 AM to 5:00 PM. There is a Preschool Program on site #191870872. LPAs observed there are separate entrances for each program and each program maintains it's own space. LPAs observed required licensing postings posted upon entry to facility including a waiver for shard use of Outdoor Play Yard with Preschool Program.

At the initial start of the site visit, there is a total census of 19 infants and a total census of 11 staff present. The facility is made up of three classrooms: RM-56, RM-51, and RM 57. LPAs observed seven infants with two teachers in RM-51, eight infants with two teachers in RM-57, and four infants with three staff for RM-56 that were playing outside during inspection.

Beginning at 9:30 AM, furniture and equipment were inspected for age appropriateness, good repair, and free of loose and sharp pointed parts in each classroom. Telephone service, heating, lighting and ventilation were evaluated. LPAs observed surface of classrooms to be safe and clean, and age appropriate tables and chairs available for infants well maintained. LPAs observed a variety of age appropriate material and equipment, including washable toys available for infants in each classroom. LPAs observed toys available for infants encourages auditory, visual, and tactile stimulation as well as manipulative skills. LPAs observed library area available for quiet play in each classroom. There were cubbies observed as storage for infants' belongings. LPAs observed low feeding chairs available for infant feeding. LPAs observed sleeping area for infants to be physically separate from activity space. LPAs observed three cribs in RM-56 for infant use and the rest of infants using age appropriate cots. LPAs observed placement of cots and cribs are not hindering walkways or work space at time of inspection. Per FR, linens and blankets are provided by the facility and

washed weekly. Availability of drinking water was reviewed. LPAs observed water coolers available for infants in each classroom, FR states that infants are provided sippy cups and disposable water cups to drink water. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPAs observed a bathroom available by entrance to Outdoor Play Area to have four toilets and three sinks available as well as a walk up diaper changing table. LPAs observed walk up diaper changing tables available in each classroom that is within arms reach of a sink with raised sides at least three inches high and with one inch padding surface that is covered with washable vinyl at time of inspection.

Snack/lunch menus were reviewed and observed posted in each classroom at time of inspection. All meals and snacks are provided by a vendor which is then prepared for distribution by Kitchen Staff. Infants are provided with AM/PM snack and Lunch. At 10:08 AM, LPAs observed Kitchen to be free of clutter and to have appropriate amount of supplies and equipment at time of inspection. LPAs observed trash cans available in each classroom to have tight fitting lids for the disposal of food waste.

A first aid kit is kept in a red backpack in each classroom. Per FR, facility has dual carbon monoxide detectors and smoke detectors, due to height placement of detectors LPAs were not able to test carbon monoxide detectors at time of inspection. LPAs observed detectors and fire extinguishers present in each classroom. LPAs observed disaster drill log available, last drill conducted on 02/15/2024. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. LPAs observed cleaning solutions to be stored on high shelves in each classroom, making these items inaccessible. FR states that there are no poisons to kill vermin stored at the facility. Per FR, there are no infants in care that require medications at time of inspection. FR states isolation area for sick infants is in Director's Office.

Beginning at 10:20 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed a designated Infant Play Yard available that is separate from Preschool Program, there is one shared Outdoor Play Area for use for Toddler and Preschool Program (waiver posted). Required shade and fencing were inspected. LPAs observed shaded area through out Outdoor Play Area and an age appropriate play structure that has a surface well maintained to absorb fall at time of inspection. LPAs observed tricycles, plastic tubes, and other play equipment available for infants to use outside that is well maintained, free of loose and sharp parts at time of inspection. within the direct care and supervision, including visual supervision of the teacher(s) at all times. The infants have access to their sippy cups during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

Teacher-Infant ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPAs observed all infants present signed in at time of inspection. Personal Rights of children were observed by LPAs.

Beginning at 11:00 AM, staff and children’s records were reviewed. Children’s Records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857. The review of Staff records was documented on the LIC 859. LPAs discussed staff having current AB 1207 Mandated Reporter Training certificate on file. LPAs observed Pediatric First Aid/CPR expires on 07/2025 for S2. All staff and employees have obtained a criminal record clearance or criminal record exemption.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC COMPLETED TESTING AND HAS REMEDIATED EXCEEDANCES
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPAs advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited only advisories given.

A notice of site visit was given and must remain posted for 30 days.
