25 | Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management Incident inspection at the above facility on 01/14/25 at 11:50 a.m. The purpose of this inspection is to follow up on an incident reported to the Department on 12/17/24. LPA met with director Kanishia “Kay” Long who guided LPA on a tour of the facility. Census was taken.
The facility began construction of the roof on 09/30/24; however, children were moved to another licensed area on campus on 09/27/24. Prior to construction, children were using the big playground; however, once construction started children began using the small playground. There were concerns regarding the dust and debris that fell from the roof onto the playground during construction on 09/30/24 which prompted the facility to conduct an asbestos test. On 11/30/24, Vert environmental conducted an asbestos test in the “small” and “big” playgrounds. Vert environmental provided the facility the asbestos contamination assessment report dated 12/11/24. Upon review of the report, LPA observed that the small playground did not test positive for asbestos; however, the big playground tested positive for asbestos. Though children are currently not using the big playground, the facility must review and follow Vert environmental recommendations. According to S1, Vert came out to decontaminate and stated that it was cleared but the facility never received a clearance letter. S1 stated that KB environmental came out on 12/18/24 measured everything but they need the Procedure-5 asbestos abatement work plan. KB cautioned the entire area and currently pending is the clean-up. S1 also stated that Vert environmental will come back though there are no outdoor guidelines. The facility is trying to figure out the outdoor guidelines to ensure everything is done accordingly. Per S1, the facility stopped all construction in late November (date unknown) and will resume later (date unknown). S1 also stated that the contaminated area in the big playground will be cleaned over the weekend, or the facility will close for approximately two days. At this moment it is unknown when the cleanup will take place since they must wait for 10 days; however, S1 assured LPA that children will not be present. During LPAs walk through on 01/14/25, LPA observed that the contaminated area (big playground) has caution tape all around to prevent access to children and staff (pictures were taken); however, the playground is not fully barricaded.
