25 | Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management- Incident investigation at the above facility on 06/15/23 at 02:15 p.m. A COVID-19 risk assessment was conducted prior to entering the facility. LPA wore appropriate personal protective equipment. LPA met with Director, Kanishia Long who guided LPA on a tour of the facility. Census was taken.
The purpose of this inspection is to follow up on an unusual incident report that occurred on 05/30/23. The facility reported this incident to the Department on 06/02/23 which was not within the required 24 hours.
During this investigation, LPA interviewed, staff #1 (S1 thru 2), children #1 (C1 thru 4), obtained a copy of facility roster and reviewed children’s files. Regarding the incident that occurred on 05/30/23, LPA will conduct a follow up required visit at a later date.
Based on the reporting requirements, the following deficiency listed on the attached LIC 809D (deficiency page) is being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative, a civil penalty of $100 can be assessed.
Exit Interview was conducted, and appeal rights were given, along with a copy of this report was provided to the Director, Kanishia Long.
