Per Reporting Party (RP), the staff members misplaced child's records.
At 10:00 AM LPA conducted childrens file reviews. In reviewing the children's files, it was discovered that 2 of the 5 files reviewed were missing forms. The facility enrolled children without obtaining necessary forms.
According to Title 22 regulations, "A separate, complete and current record for each child is maintained in the child care center." Based upon record review, the allegation that "staff misplaced day- care child's records" has been determined to be Substantiated. A finding of Substantiated means that the preponderance of evidence standard has been met. California Code of Regulations, Title 22, Division 12 Chapter 1 Article 06, Section 101221 (a) “Child's Records" is being cited.
-Pertaining to the allegation that the ''Staff did not follow day-care child's dietary restrictions.":
According to the RP, the facility did not follow child's dietary restrictions. According to to RP, C1 is allergic to sugar.
During today's inspection, LPA reviewed children's records as well as interviewed staff. During file review it was discovered that child's physician report and health history were found to be missing. Thus being unable to check for child's dietary restrictions. While conduction staff interview, staff member stated that on occasion they do give out cookies, and candy. Based upon record review and interview, the allegation that "Staff did not follow day- care child's dietary restrictions" has been determined to be Substantiated. A finding of Substantiated means that the preponderance of evidence standard has been met. California Code of Regulations, Title 22, Division 12 Chapter 1 Article 06, Section 101221 (b)(8)(a) “Child's Records" is being cited.
Notice of Site visit was given and must remain posted for 30 days.
