| Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.
***All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.
LPA observed the roof of the slide to be off of the slide. FR stated that it was loose, so it was removed to prevent injury to children in care. There is a work order in place to repair the roof of the slide. Until slide is repaired, a teacher stands in front of the slide area to ensure it is not climbed up or used during outdoor playtime.
The facility uses Care Connect to sign children in and out of the facility. LPA reviewed staff records, staff files were complete. Substitute teacher's file was not available for review, name was recorded. LPA will review staff file at district office at a later date.
Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file.
Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.
SB792 (Immunization Requirements for Staff and Employees) was discussed with the Director. The Director and staff do currently have immunization documentation.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA and FR discussed informing temporary staff and new staff of children's rights and methods for getting a child's attention and redirecting.
