On Friday, September 11, 2026 at 9:00 a.m., Licensing Staff (LS) C. Nguyen, Licensing Program Analyst (LPA) and K. Chicote, Licensing Program Manager (LPM) conducted an unannounced annual inspection and met with Director Hector Hernandez, who guided LS on a tour of the facility. Director Diana Conforti was present at a later time. This is a preschool age program that serves children ages 2 to 5 years old and has two (2) classrooms. This program operates Monday – Friday from 8:00 a.m. – 04:00 p.m. Per the Director, there are 23 children enrolled. Teacher and child ratio was observed, and present staff are fingerprint-cleared: 15 children and 4 staff; Giraffes (2-3 years old): 4 children and 1 staff, Hippos (3-4 years old) and Bears (TK 4+ years old): 11 children and 2 staff.

Sign in and out takes place at the main entrance area through the Bright Wheel App. The director stated that there are no firearms, weapons, poisons or bodies of water on the premises, and LS did not observe any of the above-mentioned. All areas identified on the facility sketch were toured and inspected both indoors and outdoors. Director stated that ill isolation takes place in the quiet/library area. LPA advised Director that there shall be a designated toilet and sink for the ill child and sanitized.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LS entered the Hippos and Bears classrooms and observed furniture, manipulatives, toys, games, and books made accessible to the children to be age-appropriate and free of sharp, loose, or pointed parts. There is drinking water readily available via and bring their own water bottles and bottles get refilled as needed via water filter pitcher. Napping equipment in the form of cots were observed in each classroom and children’s individual sheets were labeled and stored. Per Director, linen are washed by parents every month or when soiled and LPA informed Director that bedding shall be washed weekly or when soiled. Licensing staff observed step trash bins with a tight-fitting lid inside the classrooms. LS observed the restrooms, located in the classrooms to have working toilets, sinks, hand soap dispensers, and paper towels. There is a storage area in the restroom with child-safety gate to prevent children access. LS also observed the Giraffe’s classroom, separate building from Hippos and Bears. More age-appropriate furniture, toys and games were well-maintained. Giraffe’s classroom has a restroom with working sink/toilet and paper towels, and a separate sink within the classroom.

Heating, lighting, and ventilation were evaluated by LS and observed mini split AC/heater and vents located in the classrooms. LS observed the thermostats located in the classrooms and temperatures to be between 70-71 degrees. LS observed the fire extinguishers in the classroom and the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced on 10/21/25. Smoke detectors and carbon monoxide were observed and tested to be operable by Director. Director stated the last emergency drill was conducted on 8/20/2026. First aid kit supplies are kept inside the classrooms and were observed to be fully equipped.

Licensing staff observed the outdoor playground yard to be fully fenced. The outdoor play equipment was observed to be free of sharp, lose or pointed parts. Outdoor activity space surface is maintained in a safe condition and is free of hazards. Areas around and/or under climbing equipment have cushioning material to absorb a fall. There is artificial grass for one area of the playground and sand box with play structure. Outdoor maintenance are on Mondays, and LPA advise Director to do a walk-through for spiderwebs before children go outside. Drinking water is readily available outdoors. There is adequate shade via trees and shade sails.

Per Director, the facility gives a snack, and children bring lunch and snack from home. LS informed licensee that any food brought from the children's homes, shall have the container labeled with the child's name and properly stored or refrigerated. LS observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage. During time of inspection LS observed cleaning solutions stored in a locked cabinet with child-safety latches, making it inaccessible to children. Director stated that they currently do not have any children on medication or severe food allergies. Director stated there is one child with food allergies, but does not require medication.

LS reviewed the children’s roster, children’s files, staff files, and deemed it incomplete. LPA provided consultation on record keeping.

LS observed Director Critical Care Training Center Pediatric First Aid/CPR certification and expires 8/2028 and has proof of immunization against Pertussis, MMR, and Influenza declination. Director has completed the mandated reporter (AB 1207) training and expires 1/23/2028. Director was advised that the mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com.

All individuals subject to criminal background review have obtained a criminal record clearance. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES on 06/2023


Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LS verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LS discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LS also informed Director of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
