On 09/26/2023 at 3:00 PM an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and initially met with Director, Martha Ortega, who granted entry to facility and provided LPA guided tour of facility both indoors and outdoors. Facility is located on grounds of Plummer Park and currently licensed for a capacity of 34, per Director they currently have 23 enrolled. The program currently operates Monday thru Friday from 7:30 AM to 5:30 PM. The facility is made up of two classrooms: All Stars Room and Sunshine Room. Required postings were discussed and observed posted on Parent Board in each classroom.

At the initial start of the site visit, there is a total census of ten preschoolers and a total census of three staff present.

Beginning at 3:04 PM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area with soft seating available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Mats/cots were inspected. Per Director, linens are provided by the facility and washed weekly for washing. Availability of drinking water was reviewed. LPA observed water pitchers with paper cups available in each classroom. Per Director, water pitchers are refilled by water cooler in Kitchen. LPA observed water cooler and refill water jugs available at time of inspection. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed bathroom available for each classroom that had two toilets and two sinks. LPA observed a walk up diaper changing table for Sunshine Room that meet requirements and is within arms reach of a sink.

A first aid kit is kept hanging on a wall in each classroom which LPA inventoried for supplies. LPA did not observe a carbon monoxide detector to be present at the facility, per Director she had them but may have been moved by Maintenance and acknowledges there were none during today's inspection. LPA observed smoke detectors and fire extinguishers to be present in the facility. Disaster drill log was available and last drill conducted on 09/27/2023. Per Director, isolation area for sick children is in her office. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed these items stored in Staff Only area with door blocking access.

Per Director, facility provides breakfast, lunch, and PM snack. Snack/lunch menus were reviewed and observed posted. LPA observed children in Sunshine Room eating PM snack during inspection. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment.

Beginning at 3:10 PM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed two outdoor play areas to be available for preschool program. Outdoor Play Area 1 is to the right upon exit of facility, which LPA observed has turf grass, stage area, and garden. Director states this area is used for gardening and story time. Outdoor Play Area 2 is located directly in front of the facility to have short play structure with tube slides, ride along toy, and sandbox area. Director states this is is shared with the public but signs are posted on playground entrances to notify public that during specific times it is only to be used by the preschool program. LPA observed signs posted at time of inspection. LPA observed wooden mulch through out Outdoor Play Area 2's surface providing cushioning material to absorb falls. LPA observed additional playground next to Outdoor Play Area 2, which Director states is not part of preschool program. LPA observed wooden benches and chairs to be located under shaded area. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. Director states children have access to water pitchers and paper cups for drinking water outside. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out and procedures were reviewed, facility uses electronic sign in and out. LPA observed all children present today signed in at time of inspection. LPA observed those who have left for the day, which is the All Stars Room, to be signed out at time of inspection. Personal Rights of children were observed by LPA.

Beginning at 4:00 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff and copy was provided to Director during inspection. Criminal record clearances were reviewed for adults. CPR card expires on 04/01/2025 for Staff #1. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC COMPLETED SAMPLING BUT THE TEST RESULTS ARE NOT YET AVAILABLE
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA referred licensee [or facility representative] to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation. Additionally, changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Director, Martha Ortega.
