Licensing Program Analysts (LPAs) Denise Gibbs and Franchesca White conducted an unannounced annual required inspection at the above facility on 2/16/23 at 9:15 AM. LPA met with Alice Reed, Facility Representative (FR) who guided analysts on a tour of the facility.

This is a preschool program that operates Monday-Friday 7:00 A.M. - 5:00 P.M.

There were 71 children and 13 staff present when LPAS arrived. Facility capacity is in compliance License capacity and Title Five ratio guidelines. All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District.

LPAs toured Classrooms B11, B12, B13, B14 and B15. All classroom furniture was in good condition, free of loose, sharp and/or pointed parts. The floors and surfaces in the classrooms were clean and safe. Rugs were observed to lay flat on the ground to avoid tripping hazards. Water is made readily available by via water bottles labeled with the children's names. Water fountains are still not being used due to COVID-19. Per FR, there are currently children with medication. Medication is stored in the office, inaccessible to children. Medication was observed to have either a prescription label with the child's name and expiration date. Children have cubbies to store personal belongings separate from each other. Each classroom has a storage room with cots. Required water lead testing has been completed with no Action Lead Exceedance found (ALE).

LPAs toured the children’s restrooms. B-12 and B-14 share a restroom, B-11 and B-13 share a restroom and B-15 has its own restroom. Restrooms were observed to be safe and sanitary with operable sinks and toilets. LPA did observe one stall B-11/B-13 shared restroom with caution tape. Per FR, a work order has been put in to fix the toilet. Restroom has three stalls available for children.

LPAs toured the outdoor play area. Outdoor area was observed to have age appropriate toys and material for children, free of loose, sharp, and/or pointed parts. LPAs observed required cushioning under climbing structure to absorb fall. Shade was observed throughout the outdoor area and water was observed to be readily available via water bottles that are brought out from indoors. Outdoor water fountains are currently not used. LPAs observed a broken table on the grass along the gate accessible to children and a metal shovel propped up next to the art area. Per FR the items should not be there, the table will be going out for salvage. Items were made inaccessible during inspection.

LPAs reviewed Sign In/Out sheets located at the front entrance screening table. All children present were signed in with date and full signature guardian. Sign in time was discussed with FR.

LPAs observed required posted documentation which included, Facility License, Publication (PUB) 393- Notification of Parent Rights, Licensing Form (LIC) 610- Facility Disaster Plan, PUB 269- Child Passenger Restraint System, LIC 613A- Notification of Personal Rights and Lunch/Snack Menu in the lobby in the front entrance. All items observed.

Facility records were reviewed for LIC 9040- Facility Roster, 9148- Earthquake Preparedness form, and Disaster drill log, last drill conducted on 1/4/23.

Children’s records were reviewed for Emergency Card with signed medical consent, Immunization Records, LIC 995 Notification of Parents’ Rights, LIC 701- Physician’s Report, LIC 613A- Personal Rights, and signed Admissions Agreement. Children with medication have signed medication request forms and expiration dates current. All files reviewed were complete.

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse, Transcripts or Permit and current Mandated Reporter Training Certificate. All staff files reviewed were complete.

LPAs toured the kitchen located adjacent to the main office. Facility provides AM snack, PM snack and lunch. Food is not cooked at the facility. Prepackaged food is delivered daily from the neighboring elementary. Extra

food is discarded daily. Kitchen was observed to be clean, free of litter, insects and rodents. LPA observed trash cans for solid waste have tight fitting lids. Cleaning supplies are stored separate from the food. Per FR, carbon monoxide detector is wired with smoke and fire alarm.

During inspection all children were observed to be treated with dignity and respect, they were observed to be receiving safe, healthful and comfortable accommodations, furnishings and equipment, and free from corporal and/or unusual punishment.

Incidental Medical Services (IMS):
This facility provides Incidental Medical Services- IMS. LPAs reviewed storage of medication and equipment/supplies, and reviewed children’s, personal, and administrative records. Based on the LPA's observations and records review no deficiencies will be cited today 2/16/23.
