On 04/29/2024 at 9:30 am, Licensing Program Analyst (LPA), Elka Chavez conducted an announced Case Management inspection for an increase in capacity. Upon arrival, LPA met with Licensee Representative, Tanasa Davis and Maria Aguirre. This facility is currently licensed as a preschool program with a capacity of 24 children. The Licensee, Kaiser Foundation Hospital, Inc. has requested to increase the preschool capacity by 6 additional children for a total new capacity of 30 in classrooms 1 and 2.

The facility is located on the first floor of the Kaiser Permanente Watts Counseling & Learning Center. Per Facility Representative, Parents will utilize Suite 105 entrance for children enrolled in classroom 1 and 2. Hours of operation will be Monday to Friday from 7:30 am to 5:30pm. There were no children present during today’s inspection. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Tanasa Davis that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities.

LPA continued to tour the facility and measured new additional indoor and outdoor activity space. This facility was reconstructed and was completed in December of 2023. Total new indoor activity space measured 1795.82 square feet, which is sufficient to accommodate the requested increase of capacity. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via Brita water dispensers. LPA observed an observation room that can be accessed in the hallway in between both classrooms. LPA observed the ill isolation bathroom next to classroom 2. LPA did not observe a step stool or toilet potty training seat cover. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted by LA City Fire Department on 04/12/2024.

LPA observed a total of 13 sinks 5 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 30 children.

The facility currently has a fully fenced playground area. Fencing is made of brick and was observed to be at least four feet high. LPA observed wood paneling on the walls near the outdoor exit doors leading to the courtyard and side parking which leads to an alley. LPA observed splinters on the unfinished texture of the panel walls. LPA opened the door leading to the courtyard and heard an alarm alert that the door was opened. The alarms on both doors are connected to the buildings Building Management Systems. The door handles are 3 feet 6 inches from the ground and can be opened with minimal force which could be a potential health and safety risk. The total square footage for all the outdoor activity space is 3922.74 square feet, which is sufficient to accommodate the requested increase of capacity. LPA observed shade was minimal via child size picnic tables and umbrellas. There is a climbing structure on the playground for children ages 2 to 5 years of age which is properly anchored. There is adequate cushioning in fall zones of climber provided by rubber matting. LPA observed the climbing structure does not have adequate shade and the structure was hot to the touch. LPA observed the outdoor temperature was 73 degrees. LPA observed sufficient outdoor age-appropriate toys and play equipment available on the playground. Drinking water is available via water fountain. Facility Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

The following corrections are needed prior to the issuance of the license:

• Applicant needs adequate shade for the climbing structure.
• Applicant will need to ensure that the wood panels are free of sharp, loose, or pointed parts.
• Applicant will need to place a potty ring and step stool in the staff restroom/ill isolation bathroom.
• Applicant will need to ensure that the exit door handles which lead to the courtyard and side parking prevent children from leaving the outdoor activity area.

Facility representative, Tanasa Davis and Maria Aguiree understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
