Licensing Program Analyst (LPA) Franchesca White arrived at the above facility for the purpose of an unannounced random annual inspection. LPA White announced the purpose of the visit and was granted entry into the facility by Associate Director, T. Davis, and Administrator, Vikki Franklin . There are 0 children enrolled, and 0 children present during today's inspection. There are five staff members present. All staff have current criminal clearance. The hours of operation are 7:30 a.m. - 5:30 p.m. (Monday - Friday) This facility is terminating inactive status. This inspection will ensure the facility's compliance with department regulations.

All main care areas of the facility sketch were inspected for health, safety, heating and ventilation for the children. This facility has two classrooms for children ages 3-5 years of age. The facility is on the grounds of Kaiser Medical Offices and Counseling Center in suite 105. The facility ensures the safety of the children by using key cards to enter/exit every door within the facility. Upon entry of the facility the following documentation was posted: Emergency Disaster Plan, Emergency Disaster Checklists, Parent Rights, Personal Rights, Passenger Safety Law, and Shaken Baby Syndrome. Associate Director T. Davis states that parents will use Brite Wheel to electronically sign the children in and out of the facility.

At 10:00 a.m. LPA White observed classroom 1 to have cubbies for each child's personal belongings, and cots stored adjacent to the teacher's workspace. Fire extinguisher is located near the sink, and a health and safety cabinet is located near the door of the classroom. Director states that medications will be stored in the health and safety cabinet along with physician instructions for administering medication. LPA White discussed best practice of placing photo of child with medication to ensure the correct child is receiving medicine. LPA White observed first aid kit to be stored in the health and safety cabinet to be fully stocked with the following items: sterile first aid dressings, roller bandages, scissors, tweezers, thermometer, tape, antiseptic. ...............................................Report Continues 1 of 4 Pages....................................................

There are water dispensers in each classroom with paper cups available for the children.

At 10:15 a.m., LPA White observed classrooms 1 and 2 to have age-appropriate centers setup throughout the classrooms. LPA White observed the dramatic play areas to have a kitchen setup with a cabinet filled with occupational clothing for the children, grocery store, and traffic signs. LPA observed cardboard and wooden blocks, miniature people of different nationalities, and a tree house. LPA White observed a Science Area, Sensory Table, Audio Listening Space, Arts and Crafts, Writing Literacy, and the Library Reading space. LPA White observed a hand washing station of four sinks inside the classroom for the children. Classrooms 1 and 2 have accordion wall that separates each class with an observation glass providing viewing of rooms with an entry and exit door from the hallway. Director states that facility will be a teaching facility for students from nearby colleges.
LPA observed three (3) tables and chairs for the children to use for activities, and lunch in Classroom 1 and 2. At 10:25 a.m., LPA White observed that bathroom space shared by Classroom 1 and 2 to have five (5) toilets and four (4) sinks, paper towels, and a soap dispenser.

At 10:35 a.m., life saving devices such as fire extinguishers were observed to be fully charged with service tags dated 1/6/2024. Smoke detectors, and Carbon Monoxide Detectors are hard-wired into the BMS (Building Management System).

At 10:45 a.m., LPA White observed the Outdoor play area to be enclosed by a fence of concrete and wood paneling with two exit doors that are wired to alarm when opened. LPA White observed that these doors open to unfenced areas open to the public and street traffic which poses a potential hazard to children. LPA White issued a technical violation for the following violation to be corrected prior to reopening of facility. Director states that a plan of correction will be completed within 30 days. At 10:50 a.m., LPA White observed the hand washing and drinking water fountain to be in direct sunlight without sufficient shading. Twisting nozzle that turns on water was hot to the touch. At 11:00 a.m. LPA White observed the play apparatus to be hot to the touch, and without sufficient shading for children. LPA White inspected the wooden fence paneling and observed some unfinished spots that were not free of sharp, loose, or pointed parts, which could pose a potential hazard.

At 11:10 LPA White observed the kitchen and storage space. Director states that meals will be cooked on site for the children and the menus will be posted outside of each classroom. LPA observed menu postings outside of the classrooms. LPA White observed the kitchen to be free of rubbish, and sanitary for the preparation of food. LPA observed the following equipment to be in good repair at the time of inspection: dishwasher, refrigerators (2), two sinks, mixers, blenders, cutting boards, and mixing bowls. Children's plates and cups were cleaned and stored on shelves.

At 11:20, LPA White observed the Isolation area to be outside the office door of the site supervisor. A chair and folding mat were placed against a free standing wall shielding the isolation space making it inaccessible to other children in care. Director states that the Staff bathroom will be used for a sick child. Director states that a toilet ring, and stepping stool will be placed in isolation bathroom.

At 11:30 a.m., LPA White reviewed Director's file for the following documents: Mandated Reporter Training - 09/13/2025; CPR/First Aid - 08/2025; Proof of Immunization against pertussis, measles, and Influenza, TB testing, Evaluation of Director Qualifications, Transcripts, Statement of Acknowledgement of Child Abuse, and Preventative Health and Safety Certificate.

LPA White discussed the following:

Licensee [or facility representative] was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and
administrative records.

Director Vikki Franklin was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on observations, record review, and interview there are no deficiencies being cited under California Healthy and Safety Code of Title 22 regulations.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the Assistant Director T. Davis. A copy of the report, Notice of Site Visit, and appeal rights was given to T. Davis, Director.
