On 3/13/2025 at 9:45 a.m., Licensing Program Analyst (LPA) Saul Valenzuela conducted an unannounced required inspection. LPA met with Site Supervisor, Marife Adriano, and Director, Laura Hernandez who guided LPA on a tour of the facility. This is a preschool age program located across the street of the Children's Hospital Los Angeles. Facility is currently licensed for a capacity of 89 children. and operates Monday – Friday from 6:30 a.m. – 8:00 p.m. Per the Site Supervisor there are 50 children enrolled. Facility also has an infant program, license number 191806530.

All areas identified on the Facility Sketch were inspected. The following staff were present during this inspection: Toddler 3: not being used due to staff shortage; Toddlers: two staff with 9 children; P1: three staff with 9 children; P2: two staff with 10 children; Pre-K: one staff and 8 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license.

LPA observed the facility to be clean, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Napping equipment in the form of cots and bedding were inspected for good condition, appropriate storage, and cleanliness. Per Director, sheets are washed on a weekly basis or as needed. Storage for children's belongings were inspected.

All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the office is used as an isolation area. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Food menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides snacks, breakfast and lunch for all children. All food is prepared and cooked within the facility. There is drinking water available in all indoor classrooms and drinking containers / jugs are taken outdoors. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in an area inaccessible to children. Facility Representatives states there are no poisons stored at the facility.

LPA observed that there are smoke and carbon detectors in the facility and were operable on this day. Fire extinguishers were last serviced on 1/15/25, as indicated on the service tag.

The facility has a yard that is fully fenced. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. LPA observed that children from the preschool must walk through a portion of the infant yard to enter to their designated outdoor play area. According to the Site Supervisor, children from the preschool have been walking through the infant outdoor play area since they started working at the facility.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day.

Staff Records were reviewed to ensure that a health screening report is on file. LPA observed that 3 out 7 staff do not have current Mandated Reporter on file.

Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid documentation was reviewed.

Facility conducted Lead Testing of Water fixtures used for drinking or cooking. Testing had a sample date of 12/18/22 and no exceedances were reported. Page 2 of 4

Facility representative were reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. PIN 22- 05-CCP

This facility provides Incidental Medical Services - IMS. LPA reviewed storage of "medication and equipment/supplies, and reviewed children's, personnel, and administrative records. Facility representatives were informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.
