On 09/03/2025 Licensing Program Analyst (LPA) Dawn Dowling made an unannounced visit for
the purpose of delivering findings for the above allegations. LPA Dowling was greeted by Angela Massengale, Administrator and given a tour of the pre school class rooms. There were 39 children in care and 5 Teachers supervising.

During the initial course of the investigation on 5/28/2025 , LPA Dowling and LPM Navarro observed cleaning agents in the bathroom, on the counter, accessible to a child. Although Administrator put the cleaning agents away while LPA Dowling and LPM Navarros were at the facility the allegation is Substantiated and will result in a Type B deficiency. Upon LPA Dowling inspected the bathroom observed a can of Lysol on the counter and accessible to children in care. Based on LPA's observation the above allegation is substantiated. There is evidence to show that cleaning agents were left out and accessible to children in care.

Based on LPA's observation, the licensee did not comply with the section cited above. During a tour of the bathroom, LPA observed that the bathroom counter had disinfected out on the counter and was not put up out of a child's reach.

Upon entering bathroom LPA Dowling noticed that chemicals were on the counter, within reach of children. Additionally, LPA Dowling and LPM Navarro observed chemical's on counter on 5/28/25 which were a risk to persons in care.
