25 | On August 16, 2023, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced case management inspection at the above facility. A COVID-19 risk assessment was conducted prior to entering the facility. The purpose of the inspection is to follow up on an unusual incident that occurred on 07/10/2023. The incident was reported to the department on 07/11/2023; the report was submitted in a timely manner. LPA met with Principal, Imelda Mendez who guided LPA on a tour of the facility. LPA observed 43 children in care.
Brief Summary: On July 10, 2023 at 3:35pm, Parent #1 (P1) spoke to Staff #1 (S1), and stated that Child #1 (C1) told P1 that the "teacher did pow! pow!" to C1. P1 was unable to provide more details on the incident.
During the inspection LPA observed staff files, C1's file, obtained a copy of facility roster, interviewed S1-S3, and attempted to interview C1. C1 did no qualify to be interviewed.
Based on interviews conducted with Staff #1 - Staff #3 they stated that they have never observed a staff member violate a child's personal rights and they have never violated a child's personal rights. LPA asked S1 if P1 had given any more information as to what teacher may have hit C1; S1 stated that P1 did not know and did not provide any more information.
LPA provided Child's Personal Rights Regulation to Principal.
The facility is not being cited any deficiencies today, 08/16/2023.
An exit interview was conducted and a copy of this report was provided to Principle, Imelda Mendez along with Notice of Site Visit. |
