32 | During the interview, the Program Director stated that she first became aware that the door to Pre-K Classroom 1 was in disrepair sometime during the previous week but was unable to recall the exact date. She explained that because the building is owned by the Housing Authority, the center is required to follow an established protocol for repairs. When a maintenance issue is identified, the center reports the concern to the Housing Authority, which is responsible for dispatching a technician to complete the repair. The Program Director stated that after no technician responded by Friday (7/31/26), she contacted the Housing Authority on Monday afternoon (8/3/26) via email and text message to report that the door to Pre-K Classroom 1 was in disrepair and could not be opened from the outside. On the morning of 8/4/26, a follow up email was sent requesting that a technician inspect and repair the door. During today's inspection, the LPA tested the door handle to Pre-K Classroom 1 and observed that the door could not be opened from the outside. However, a teacher was able to open the door from the inside without difficulty to exit the classroom. The classroom also has two additional exit doors located on the opposite side of the room. The LPA tested both doors and observed that they were fully operational from both the inside and outside. Based on these observations, exiting Pre-K Classroom 1 would not present a hazard in the event of an emergency.
During today's visit, a technician arrived at the center and informed the LPA that the door handle to Pre-K Classroom 1 would be repaired by the end of business day on 8/5/26. Although the door handle to Pre-K Classroom 1 was observed to be in disrepair, the Program Director took appropriate steps to request repairs within a reasonable time frame, taking into consideration the required maintenance protocol established by the Housing Authority, which owns the building. Additionally, no immediate health or safety hazard was identified, as the door remained operable from the inside and the classroom had two additional functioning exit doors available for emergency egress.
Based on the investigation conducted, there is insufficient evidence to support the above mentioned allegation to be true. Therefore, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.
The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
