On 2/28/2025 at 9:00am Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced annual/random inspection. LPA entered the facility and met with Front Desk Receptionist Katherine Castillo. Shortly after LPA met with Site Supervisor Lucy Bryant.

Upon entrance LPA Calderon was provided with Children's Roster, Staff Roster, updated LIC398 Designation of Responsibility, and updated LIC610 Emergency Disaster Plan.

Licensing Program Manager Denise Gibbs arrived at the facility at approximately 9:25am. LPA Calderon and LPM Gibbs toured the facility alongside Lucy Bryant. The facility is Licensed to serve a capacity of 73 children who are Ambulatory and Licensee prefers to serve ages 2 thru 6 years old.

This facility consists of 4 classrooms, facility has a room they call Toddler Room that consist of children ages 2 years old, facility does not have Licensed Toddler Component. The operating hours for this facility are Monday through Friday from 7:30 AM to 5:00 PM. All areas identified on the Facility Sketch were inspected.

The following was inspected and observed during the facility tour: Room 1A: 2 staff with 11 preschoolers; Room 1B: 3 staff with 14 preschoolers; and Room 2: 2 staff with 9 preschoolers. The ' Toddler Room' with 2 year old preschoolers had 4 staff and 7 children. The facility was observed to be within the license capacity and limitations. Rooms 1A and 'Toddler Room' have staircase, loft is used for classroom storage, stairs are blocked off by baby safety gates to provide inaccessibility.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed by the facility once a week, each classroom has its scheduled day, laundry machines observed to be in a back room in the kitchen area. In 'Toddler Room' LPA observed linen left on each child's cot stacked up on one another having bedding be in contact with other beddings, picture taken and deficiency cited. LPA observed age appropriate sinks and toilets were inspected for availability and good repair in all classrooms. General sanitation was observed.

Facility gets water delivery service from the company: Sparkeltte, water dispenser available by school entrance. Water gets delivered every 2 weeks per Site Supervisor. Classrooms 1B and Room 2 have a pitcher readily available in the classroom. Toddler Room water pitcher was in a refrigerator outside of the classroom, LPA consulted with staff in the classroom stating water needs to be readily available for children to drink, Staff immediately brought over the water pitcher into the classroom. Classroom 1A had water bottles for children, extra water bottles and use water dispenser when needed. Per Site Supervisor stated classrooms bring out cups and water pitcher outside when playing outdoors. During the inspection, lead testing postage was posted on the bulletin board in the facility lobby area. Fountains at the facility, were observed not to have running water.

Disinfectants and cleaning solutions dangerous to children were stored in each classroom in high cabinets above the sinks. Janitor room has cleaning solutions, room is maintained closed and per Site Supervisor children pass by room only with supervisor of a teacher and/or guardian. LPA gave consultation to make room more inaccessible. Per site supervisor will keep room locked. Carbon monoxide detector was observed in kitchen room, was tested and found operable. Smoke alarm and Fire alarms are interconnected and hardwired with Los Angeles Fire Department. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans throughout the school used to discard food and/or solid waste have tight fitting lids or solid waste bags. Facility has food menu on the bulletin board by each classroom entrance. School gets there food cooked and food delivered by sister school Rita Child Dev. Center. Per Site Supervisor children with food allergies have modified diets and LPA reviewed documentation. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shaded areas in the play yard. There are no firearms stored on the premises. There are no bodies of water at the facility.

(5) Staff records were reviewed for qualifications, Education, Criminal Record Statement, training's and vaccinations. All staff present at the facility are fingerprinted cleared. There is at least one person trained in CPR and Pediatric First Aid present during this inspection, Site Supervisor pediatric first aid and cpr expires 1/4/2027. Site Supervisor mandated reporter expires 4/25/25. Facility uses third-party Child-Care Careers for clearance of substitute teachers. First Aid supplies were observed in the classroom. According to site supervisor, facility does not currently administer any medication at this time. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in facility nurse room.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. CCC COMPLETED TESTING.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was cited and provided a deficiency on LIC 809-D, Per California Code of Regulation Title 22, one (1) Type B was cited on today's visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Lucy Bryant.
